MPT与MGT在ABO新生儿溶血病患儿输血前检验中的价值

摘要 目的 探讨微孔板凝聚胺技术(MPT)与微柱凝胶技术(MGT)在ABO新生儿溶血病患儿输血前检验中的价值。方法 随机选取50例ABO新生儿溶血病患儿为研究对象,分别采用MPT与MGT对患儿的血液标本进行输血前检验。分析ABO新生儿溶血病患儿与母体血型鉴定结果 ,患儿血清和放散液交叉配血与直抗类型情况,放散液MPT和MGT配血结果 ,血清MPT和MGT配血结果。结果 母体血型检测结果均为O型血,共计27例患儿血型为A型、占比为54%,共计23例患儿血型为B型、占比为46%。50例患儿血清交叉配血252次,血清MGT检测阳性130次(51.59%),放散液MGT检测阳性92次(36.51%),血清MPT检测阳性26次(10.32%),放散液MPT检测阳性27次(10.71%)。两种方法检测阳性率比较,差异具有统计学意义(P<0.05)。放散液MPT检测结果为阳性时MGT均为阳性, Kappa一致性检验结果表明MPT和MGT的放散液交叉配血结果一致性较差(K=0.201, P>0.05)。血清MPT检测结果为阳性时MGT检测结果均为阳性, Kappa一致性检验结果表明MPT和MGT的血清交叉配血结果一致性较差(K=0.303, P>0.05)。结论 MGT在ABO新生儿溶血病患儿输血前检验中的敏感度和准确度均较高,利于保障输血安全。 Objective To discuss the value of manual polybrene test(MPT)and microcolumn gel test(MGT)in pre-transfusion testing of neonates with ABO hemolytic disease.Methods 50 neonates with ABO hemolytic disease were randomly selected as the study subjects,and their blood samples were tested by MPT and MGT respectively before transfusion.The results of blood group identification between the neonates with ABO hemolytic disease and their mothers were analyzed,as well as cross-matching and type of direct antiglobulin of the children's serum and elution fluid,matching results of MPT and MGT in elution fluid,and matching results of MPT and MGT in serum.Results The blood type of the mother was blood type O,a total of 27 children were blood type A,accounting for 54%,and a total of 23 children were blood type B,accounting for 46%.In 50 children,252 times of serum cross-matching were performed,including 130 times(51.59%)of serum MGT positive,92 times(36.51%)of MGT positive in elution fluid,26 times(10.32%)of serum MPT positive,and 27 times(10.71%)of MPT positive in elution fluid.The positive rate of the two methods was compared,and the difference was statistically significant(P<0.05).MGT was positive when MPT was positive in elution fluid,and Kappa consistency test showed that the cross-matching results of MPT and MGT in elution fluid were not consistent(K=0.201,P>0.05).MGT was positive when serum MPT results were positive,and Kappa consistency test results showed that the consistency of serum cross-matching results of MPT and MGT was poor(K=0.303,P>0.05).Conclusion MGT has high sensitivity and accuracy in pre-transfusion testing of neonates with ABO hemolytic disease,which is conducive to the safety of blood transfusion.
