复方黄芪汤联合骨化三醇治疗糖尿病肾病的疗效观察

Observation of efficacy of compound Huangqi decoction combined with calcitriol on diabetic nephropathy

目的分析糖尿病肾病(DN)特点,评价复方黄芪汤与骨化三醇联合治疗的预后效果。方法92例DN患者,随机分为对照组与观察组,各46例。对照组采用骨化三醇治疗,观察组在对照组基础上联合复方黄芪汤治疗。对比两组治疗效果、治疗前后血糖[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]与肾功能指标[血肌酐(Scr)、尿素氮(BUN)、尿白蛋白(ALB)]水平。结果观察组治疗总有效率95.65%高于对照组的78.26%,差异有统计学意义(P<0.05)。治疗后,观察组FBG(6.50±0.80)mmol/L、2 h PG(9.75±1.05)mmol/L、HbA1c(5.99±1.06)%、Scr(83.30±8.20)μmol/L、BUN(6.42±0.80)mmol/L、尿ALB(0.35±0.10)mg/ml水平均明显优于对照组的(7.45±1.50)mmol/L、(11.30±1.20)mmol/L、(7.45±1.20)%、(88.80±8.50)μmol/L、(7.50±1.02)mmol/L、(0.88±0.15)mg/ml,差异均有统计学意义(P<0.05)。结论复方黄芪汤联合骨化三醇治疗DN效果显著,有助于控制患者血糖水平、改善肾功能。

Objective To analyze the characteristics of diabetic nephropathy(DN),and evaluate the prognostic effect of compound Huangqi decoction and calcitriol combined treatment.Methods A total of 92 patients with DN were randomly divided into control group and observation group,with 46 cases in each group.The control group was treated with calcitriol,and the observation group was treated with compound Huangqi decoction on the basis of control group.The therapeutic effect,blood glucose[fasting blood glucose(FBG),2 h postprandial glucose(2 h PG),glycosylated hemoglobin(HbA1c)]and renal function index[serum creatinine(Scr),blood urea nitrogen(BUN),urinary albumin(ALB)]before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 95.65%,which was higher than that of 78.26%in the control group,and the difference was statistically significant(P<0.05).After treatment,the observation group had FBG of(6.50±0.80)mmol/L,2 h PG of(9.75±1.05)mmol/L,HbA1c of(5.99±1.06)%,Scr of(83.30±8.20)μmol/L,BUN of(6.42±0.80)mmol/L,and urinary ALB of(0.35±0.10)mg/ml,which were obviously better than those of(7.45±1.50)mmol/L,(11.30±1.20)mmol/L,(7.45±1.20)%,(88.80±8.50)μmol/L,(7.50±1.02)mmol/L,and(0.88±0.15)mg/ml in the control group,and the differences were statistically significant(P<0.05).Conclusion Compound Huangqi decoction combined with calcitriol has a significant effect on the treatment of DN,which can help control the patient's blood glucose level and improve renal function.
