摘要
目的评估多媒体教学视频+超声模拟培训+以问题为基础的学习(problem-based learning,PBL)教学法在麻醉专业住院医师桡动脉穿刺置管中的教学效果及优点。方法选择2020年8月—2021年8月首都医科大学宣武医院进行住院医师规范化培训的24名麻醉专业学员,随机分为对照组和试验组,每组12名。对照组住院医师采用传统法行桡动脉穿刺置管教学;试验组住院医师采则用多媒体教学视频+超声模拟培训+PBL教学法进行教学。记录并分析两组住院医师在5次临床操作中的考核结果及问卷调查评价。结果试验组住院医师在操作考核中的物品准备漏项(1.25±0.92)项、成功置管所需穿刺次数(1.92±0.35)次分别显著低于对照组的(2.69±1.16)项及(2.36±0.33)分,而操作规范化得分(80.28±6.11)分则显著高于对照组(66.94±6.85)分,差异有统计学意义(P<0.05);试验组住院医师在问卷调查各项评分(教学方法、学习积极性、学习效率、理论知识记忆效果、临床操作能力提高)均显著高于对照组,差异有统计学意义(P<0.05)。结论多媒体教学视频+超声模拟培训+PBL教学法在麻醉专业住院医师桡动脉穿刺教学中的效果优于传统教学法。
Objective To evaluate the teaching effect and advantages of multimedia teaching video+ultrasound simulation training+problem-based learning(PBL)teaching method in radial artery puncture catheterization for anesthesia residents.Methods Anesthesia residents who underwent standardized training in Xuanwu Hospital of Capital Medical University from August 2020 to August 2021 were randomly divided into control group and experimental group,with 12 residents in each group.The residents in the control group were taught radial artery puncture and catheterization by traditional method.Residents in the experimental group were taught with multimedia teaching video+ultrasound simulation training+PBL teaching method.The assessment results and questionnaire evaluation in 5 clinical operations of the two groups were recorded and analyzed.Results The items missing in preparation and the number of punctures required for successful catheterization in the experimental group were(1.25±0.92)and(1.92±0.35)points,respectively,which were significantly lower than(2.69±1.16)and(2.36±0.33)points in the control group,the standardized operation score of the observation group(80.28±6.11)points was significantly higher than that of the control group(66.94±6.85)points,and the differences were statistically significant(P<0.05).The scores of questionnaire survey(teaching methods,learning enthusiasm,learning efficiency,theoretical knowledge memory effect,clinical operation ability improvement)in the experimental group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Multimedia teaching video+ultrasound simulation training+PBL teaching method is better than traditional teaching method in radial artery puncture teaching of anesthesia residents,which is worthy of promotion.
