手法复位结合克氏针固定治疗翻转型儿童肱骨外髁骨折的效果

Effect of manipulative reduction combined with Kirschner wire fixation in the treatment of children with overturned humeral external condyle fracture

目的探讨手法复位结合克氏针固定治疗翻转型儿童肱骨外髁骨折的效果。方法选择2021年9月至2022年5月临沂市兰山区枣园中心卫生院收治的80例肱骨外髁骨折患儿为研究对象,按照随机数字表法分为两组,各40例。观察组实施手法复位结合克氏针固定治疗,对照组则于全身麻醉下实施骨折切开复位钢板内固定术。比较两组手术指标,如提携角丢失角度和肘关节屈伸受限情况,统计两组术后换药次数及骨折愈合时间,记录两组术后并发症发生情况。结果观察组术后提携角丢失角度和肘关节屈伸受限角度小于对照组(P<0.05)。观察组术后换药次数少于对照组(P<0.05),骨折愈合时间早于对照组(P<0.05)。观察组发生伤口感染、内固定松动和畸形愈合的总比例低于对照组(P<0.05)。结论手法复位结合克氏针固定治疗翻转型儿童肱骨外髁骨折,手术效果理想,可有效促进术后恢复,减少并发症,保障治疗效果。

Objective To explore the effect of manipulative reduction combined with Kirschner wire fixation in the treatment of children with overturned humeral external condyle fracture.Methods A total of 80 children with overturned humeral external condyle fracture were selected as t he research objects from September 2021 to May 2022 in Lanshan Zaoyuan Central Hospital of Linyi City and they were divided into two groups according to the random number table method,40 cases in each group.The observation group was treated with manipulative reduction combined with Kirschner wire fixation,while the control group was treated with open reduction and plate internal fixation under general anesthesia.The surgical effect indicators,such as carrying an gle’s lost angle and elbow flexion and extension limitation,were compared between the two groups,and the number of dressing changes after operation and the follow-up time of fracture healing were counted,the occurrence of postoperative complications was recorded between the two groups.Results In the observation group,the angle of carrying angle loss after operation and the angle of elbow flexion and extension were smaller than those in the control group(P<0.05).The number of dressing changes in the observation group was fewer than that in the control group(P<0.05),and the fracture healing time was earlier than that in the control group(P<0.05).The total rate of wound infection,loose internal fixation and abnormal healing occurring in the observation group was lower than that in the control group(P<0.05).Conclusion Manipulative reduction combined with Kirschner wire fixation for the treatment of children with overturned humeral external condyle fracture has an ideal surgical effect,which can effectively promote postoperative recovery,reduce complications and ensure the therapeutic effect.
