2017—2019年遂宁市恶性肿瘤发病和死亡分析 被引量:1

Analysis of cancer incidence and mortality in Suining from 2017 to 2019

摘要 目的了解遂宁市恶性肿瘤发病、死亡流行特征及常见癌症,为防治策略提供参考依据。方法在中国肿瘤登记平台导出遂宁市2017-2019年恶性肿瘤新发病、死亡病例,采用SPSS 25.0软件对结果进行统计分析,使用t检验、χ^(2)检验分别进行均数、率的比较,以P<0.05为差异有统计学意义。结果2017-2019年遂宁市粗发病率255.61/10万,中标率147.99/10万,3年中标率分别为141.62/10万、150.08/10万和154.98/10万,发病前10位是肺癌、胃癌、肝癌、食管癌、直肠癌、乳腺癌、结肠癌、子宫颈癌、脑(中枢神经系统)癌和胰腺癌;粗死亡率171.35/10万,中标率91.80/10万,3年中标率分别为98.41/10万、94.86/10万和90.40/10万,死亡前10位是肺癌、肝癌、胃癌、食管癌、直肠癌、脑(中枢神经系统)癌、胰腺癌、结肠癌、其他的或未指明部位癌和乳腺癌。2017-2019年新发病例按生存、死亡分组单因素分析,年龄、性别、职业、教育、婚姻差异均有统计学意义(P均<0.001),老年、男性、丧偶等为恶性肿瘤死亡危险因素;多因素Logistic回归分析结果显示,年龄、性别、职业、教育、婚姻均为遂宁市恶性肿瘤病例死亡独立影响因素,随着年龄每增大1岁,死亡风险增加0.036倍;女性死亡风险小于男性;农、林、牧、渔业生产及辅助人员死亡风险大于党的机关、国家机关、群众团体和社会组织人员;受教育程度高者死亡风险小于教育程度低者;已婚者死亡风险小于未婚者。结论遂宁市2017-2019年恶性肿瘤发病率逐年上升,死亡率逐年下降;常见癌症为肺癌、胃癌、肝癌、食管癌、直结肠癌、乳腺癌等,应针对常见癌症强化防治措施;胃癌发病高于全省,需加强关注。 Objective To understand the epidemiological characteristics of incidence and mortality of malignant tumors and common types of cancer in Suining City,and to provide the reference for prevention and control strategies.Methods The incidence and death cases of malignant tumors in Suining city from 2017 to 2019 were exported on the China Cancer Registration Platform.The results were statistically analyzed by using SPSS 25.0,the mean and rate were compared by using t test andc2 test.The difference was statistically significant with P<0.05.Results The crude incidence rate from 2017 to 2019 was 255.61/105,and the age-standardized incidence rates by Chinese standard population was 147.99/105.The three-year age-standardized incidence rates by Chinese standard population were 141.62/105, 150.08/105 and 154.98/105 respectively. The top 10cancers were lung cancer, gastric cancer, liver cancer, esophageal cancer, rectal cancer, breastcancer,colon cancer,cervical cancer,brain (central nervous system),and pancreatic cancer. Thecrude mortality rate was 171.35/105, and the age- standardized rate by Chinese standard populationwas 91.80/105. The three- year age- standardized mortality rate by Chinese standard population were98.41/105,94.86/105 and 90.40/105,respectively. The top 10 deaths were lung cancer,liver cancer,stomach cancer, esophageal cancer, rectal cancer, brain (central nervous system), pancreaticcancer, colon cancer, other or unspecified sites, and breast cancer. Single factor analysis ofsurvival and death grouping of new cases in 2017 to 2019, there were significant differences inage, sex, occupation, education and marriage (P<0.001), the risk factors for malignant tumordeath were elderly,male,and widowed;Multivariate logistic regression analysis showed that age,sex, occupation, education and marriage were all independent influencing factors of malignanttumor death in Suining, The risk of death increases 0.036- fold with each year of age increase;Women had a lower risk of death than men;The risk of death for agricultural, forestry, animalhusbandry,fishery production and auxiliary personnel was greater than that of personnel from Partyorgans,state organs,mass organizations,and social organizations;The risk of death for those withhigh education is lower than for those with low education;Married people have a lower risk ofdeath than unmarried people. Conclusions From 2017 to 2019, the incidence rate of malignanttumors in Suining City increased year by year,and the mortality rate decreased year by year;Thecommon cancers are lung cancer, gastric cancer, liver cancer, esophageal cancer, colorectalcancer,breast cancer,etc. The prevention and control measures should be strengthened for commoncancers;The incidence of gastric cancer is higher than that of the whole province,requiring moreattention.
