不同体质量指数的不孕不育患者激素水平与胚胎质量参数的相关性

Relationship between hormone level and embryo quality parameters in infertile patients with different body mass index

摘要 目的通过探讨不同体质量指数(BMI)不孕不育患者的激素水平与胚胎质量参数的相关性,为临床干预促排卵提供一定的参考依据。方法收集2017年6—2019年9月在本中心行IVF-ET的不孕不育患者资料进行回顾性分析。根据BMI指数将其分为≥24(n=54)和18.5~23.9(n=64)两组,再分别统计两组患者FSH、LH、E2水平以及获卵数、MⅡ卵数、正常受精卵数、优质胚胎数与可利用胚胎数,然后分析两组数据之间的相关性,最后统计两组临床妊娠率。结果两组数据BMI指数之间存在统计学差异(P<0.05);两组数据FSH之间存在统计学差异(P<0.05);两组激素水平LH、E2以及各项胚胎质量指标之间均无统计学差异(P>0.05);当BMI≥24时,FSH与获卵数之间存在负相关(P<0.05);当BMI在18.5~23.9之间时,FSH与获卵数之间呈负相关性(P<0.05);FSH与MII卵数存在负相关(P<0.05);两组妊娠结局之间无统计学差异(P>0.05)。结论BMI可能通过影响性激素FSH水平进而影响获卵数与卵子成熟率,因此在研究胚胎发育质量时要充分考虑BMI指数对它的影响,但BMI对妊娠结局的影响仍需要更多的实验研究去验证。 Objective To explore the correlation between hormone levels and embryo quality parameters in infertile patients with different body mass index(BMI).Methods From June 2017 to September 2019 in the center of the implementation of IVF-ET infertility patients data were retrospectively analyzed.According to BMI,the patients were divided into 24 or more(n=54)and 18.5~23.9(n=64)in two groups.The levels of FSH,LH and E2,as well as the number of eggs harvested,the number of MⅡeggs,the number of normal fertilized eggs,the number of high-quality embryos and the number of available embryos in the two groups were counted respectively.The correlation between the two groups of data was analyzed,and finally the clinical pregnancy rate of the two groups was calculated.Results There was a statistical difference in BMI between the two groups(P<0.05).There was a statistical difference between the two groups in FSH(P<0.05).There was no statistical difference between the hormone levels of LH,E2 and various embryo quality indicators between the two groups(P>0.05).When BMI≥24,there was a negative correlation between FSH and the number of eggs obtained(P<0.05).When BMI was between 18.5-23.9,there was a negative correlation between FSH and the number of eggs acquired(P<0.05).There was no statistical difference in pregnancy outcome between the two groups(P>0.05).Conclusion BMI may affect the number of eggs acquired and the maturation rate of eggs by affecting the level of sex hormone FSH.Therefore,the influence of BMI on embryo development quality should be fully considered,but more experimental studies are needed to verify the influence of BMI on pregnancy outcome.
