摘要
目的探讨国产枸橼酸咖啡因的早期使用对胎龄小于34周早产儿预后的影响。方法将南充市中心医院2018年1月—2019年10月的胎龄小于34周的120例早产儿按随机原则分为早期组和晚期组,早期组在生后24 h内开始使用国产枸橼酸咖啡因;晚期组在生后24 h以后才开始使用国产枸橼酸咖啡因,通过临床资料数据统计分析两组咖啡因使用天数、给氧时间、呼吸机辅助通气以及支气管肺发育不良(BPD)、动脉导管未闭(PDA)、脑室周围-脑室内出血(PIVH)、早产儿呼吸暂停(AOP)的发生率。结果早期组与晚期组相比,枸橼酸咖啡因使用天数及呼吸机辅助通气时间、需氧时间、BPD及AOP发生率差异有统计学意义(P<0.05),而PIVH、PDA的发生率差异无统计学意义(P>0.05)结论国产橼酸咖啡因在胎龄小于34周早产儿24小时内使用,在咖啡因使用天数、用氧需求、呼吸机辅助通气及BPD、AOP发生率方面优于晚期使用,有利于改善早产儿的预后结局。
Objective To investigate the effect of the early use of domestic caffeine citrate on the prognosis of gestational age less than 34 weeks.Methods A total of 120 premature infants with gestational age less than 34 weeks in Nanchong Central Hospital from January 2018 to October 2019 were randomly divided into early group and late group.The early group began to use domestic caffeine citrate within 24 h after birth.The late group did not start to use domestic caffeine citrate until 24 h after birth.Clinical data through data statistical analysis of two groups of caffeine use days,give oxygen time,breathing machine auxiliary ventilation and bronchial pulmonary dysplasia(BPD),arterial catheter was not closed around(PDA)and ventricular-intraventricular hemorrhage(PIVH),the incidence rate of premature infant apnea(AOP).Results Compared to the early and late group,citrate caffeine use breathing machine auxiliary ventilation and number of days of time,aerobic time,difference in incidence of AOP and BPD(P<0.05),and the incidence of PIVH,PDA there was no statistically significant difference(P>0.05).Conclusion Domestic caffeine citrate was used within 24 hours in premature infants with gestational age less than 34 weeks,which was better than late use in caffeine use days,oxygen requirement,ventilator assisted ventilation and incidence of BPD and AOP,which was conducive to improving the prognosis of premature infants.
