血清HbA1c、GSP、FPG、TC、TG及CRP联合检测在早期妊娠糖尿病诊断中应用研究

Application of Combined Serum HbA1c,GSP,FPG,TC,TG and CRP Tests in the Diagnosis of Early Gestational Diabetes Mellitus

目的分析血清糖化血红蛋白(HbA1c)、糖化血清蛋白(GSP)、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)与C反应蛋白(CRP)联合检测诊断早期妊娠糖尿病(gestational diabetes mellitus,GDM)的效能。方法选取2019年4月—2022年4月龙岩市第一医院收治的58例早期GDM孕妇纳入观察组,另将同期于本院行体检的52名健康孕妇纳入对照组。采集两组静脉血,对比两组HbA1c、GSP、FPG、TC、TG及CRP差异;另绘制受试者工作曲线(ROC),探究各项指标单独与联合检测诊断GDM的效能。结果观察组HbA1c(7.63±1.52)%、GSP(3.40±0.52)mmol/L、FPG(6.35±1.75)mmol/L、TC(5.79±1.23)mmol/L、TG(1.98±0.46)mmol/L、CRP(9.23±1.56)mg/L高于对照组[(5.48±1.03)%、(1.89±0.31)mmol/L、(4.12±1.01)mmol/L、(4.52±0.85)mmol/L、(1.42±0.25)mmol/L、(4.21±0.63)mg/L],差异有统计学意义(P<0.05);ROC结果显示HbA1c、GSP、FPG、TC、TG、CRP联合检测的曲线下面积(AUC)高于各项单独检测。结论HbA1c、GSP、FBG、TC、TG、CRP在早期GDM孕妇机体内呈高表达状态,各项联合检测可有效提高疾病的早期检出率。

Objective To analyze the efficacy of combined serum glycated hemoglobin(HbA1c),glycated serum protein(GSP),fasting plasma glucose(FPG),total cholesterol(TC),triglyceride(TG)and C-reactive protein(CRP)tests in the diagnosis of early gestational diabetes mellitus(GDM).Methods 58 pregnant women with early GDM admitted to Longyan First Hospital from April 2019 to April 2022 were included in the observation group,and 52 healthy pregnant women who underwent physical examination in the same period were included in the control group.The venous blood of the two groups was collected and the differences of HbA1c,GSP,FPG,TC,TG and CRP between the two groups were compared.In addition,the subject working curve(ROC)was drawn to explore the efficacy of various indicators in the diagnosis of GDM alone and jointly.Results HbA1c(7.63±1.52)%,GSP(3.40±0.52)mmol/L,FPG(6.35±1.75)mmol/L,TC(5.79±1.23)mmol/L,TG(1.98±0.46)mmol/L,CRP(9.23±1.56)mg/L of the observation group were higher than the those of the control group[(5.48±1.03)%,(1.89±0.31)mmol/L,(4.12±1.01)mmol/L,(4.52±0.85)mmol/L,(1.42±0.25)mmol/L,(4.21±0.63)mg/L],the difference was statistically significant(P<0.05).ROC results showed that the area under the curve(AUC)of HbA1c,GSP,FPG,TC,TG and CRP combined detection was higher than that of the single detection.Conclusion HbA1c,GSP,FPG,TC,TG,and CRP are highly expressed in the organism of pregnant women with early GDM,and the combined detection of these tests can effectively improve the early detection rate of the disease.
