摘要
[目的]分析反流性食管炎(reflux esophagitis,RE)内镜下分级及合并食管裂孔疝(hiatus hernia,HH)临床特征。[方法]回顾分析1005例RE患者的临床特征,包括年龄、性别、合并HH情况。并进一步分析RE与HH的关系。[结果]RE男性多于女性,RE内镜下分级A级最常见(62.09%),不同年龄比较差异有统计学意义(P=0.000)。合并HH者60例(5.97%),未合并HH者945例(94.03%),前者年龄明显高于后者(P=0.000)。≤30岁RE患者合并HH者明显低于未合并HH者(P=0.009),>60岁RE患者合并HH者明显高于未合并HH者(P=0.013)。RE中A级未合并HH者明显高于合并HH者(P=0.000);但B、C、D级中合并HH者明显高于未合并HH者,HH发生与年龄增大有关(P=0.003,0.000,0.000)。[结论]RE内镜下分级A级最常见;RE与性别及年龄有关;RE严重程度与HH相关。
[Objective]To analyze the clinical features of reflux esophagitis(RE)in endoscopic classification and the relationship with hiatal hernia(HH).[Methods]The clinical features of 1005patients with RE were retrospectively analyzed.The relationship between RE and HH was further analyzed.[Results]Grade A was most common in RE(62.09%).There was a significant difference in age between different levels of esophagitis(P=0.000).There were 60cases(5.97%)of HH patients and 945cases(94.03%)of those without HH,and the age of the former was significantly higher than that of the latter(P=0.000).Rate of≤30-year-old RE patients with HH were significantly lower than those without HH(P=0.009),and patients with RE more than 60years were significantly higher than those without HH(P=0.013).The Grade A without HH in RE was significantly higher than that combined HH,and the difference was statistically significant(P=0.000);However,the combined HH in Grades B,C and D was significantly higher than that without combined HH,and the difference was statistically significant(P=0.003,0.000,0.000).[Conclusion]Grade A of RE is the most common under endoscopy.The gender and age have a relationship with RE.The severity of RE is related with HH.
