摘要
目的本研究意在对比助听器REM验配法与快速验配法对患者言语理解力和满意度等方面的影响。方法本研究对27位助听器患者进行研究,其中男性16例,女性11例,患者平均年龄67.67±17.59岁。采用配对样本的Wilcoxon检验对REM验配方法及快速验配方法所得频响差值、SII、IOI-HA调查问卷调查结果进行分析,P<0.05具有统计学意义。采用非参数方法Kruskal-Wallis检验分析性别、年龄、助听器使用经验、外形、通道数因素对患者言语识别率效果的影响,时间节点分别为:未助听、首次快速验配、REM首次匹配、REM二次匹配的结果进行统计,P<0.05具有统计学意义。结果Wilcoxon检验结果显示,在频率范围250Hz~4000Hz内,除1000Hz强度为65dB SPL与80 dB SPL的P>0.05(结果未见统计学差异),其他频率50、65和80 dB SPL强度下的P结果均小于0.05。结果具有统计学差异;SII、IOI-HA问卷的调查结果显示REM验配方法得分均值高于快速验配方法得分均值。患者在四个时间节点SRS得分均值逐渐增加,性别、年龄、助听器使用经验、外形、通道数因素对患者言语识别率无统计学显著差异(P>0.05);结论临床使用REM验配方法进行REAR的测试并按照目标公式匹配调试助听器,比快速验配方法为患者带来更高的言语可懂度及满意度。
Objective To compare speech comprehension and satisfaction outcomes between real ear measurement(REM)verified fitting and quick fitting.Methods REM fitting and quick fitting were tested in 27 patients(16 males and 11 females,average age=67.67±17.59 years).Outcome measures included frequency response difference,SII and IOI-HA questionnaires and were compared using the Wilcoxon test.Effects of gender,age,hearing aid use experience,hearing aid appearance and number of channels on speech recognition outcomes were also analyzed using the non-parametric Kruskal-Wallis test.Results Were collected and compared before fitting,at first quick fitting,first REM fitting and secondary REM fitting.Results In the frequency range of 250–4000 Hz,outcomes showed significant differences between REM and quick fitting(P<0.05),except for 1000 Hz at 65 and 80 dB SPL(P>0.05).SII and IOI-HA scores were higher for REM fitting than for quick fitting.The mean SRS score steadily improved across the four time nodes.Gender,age,hearing aid use experience,hearing aid appearance and number of channels showed no effects on speech recognition outcomes(P>0.05).Conclusion REM fitting to the target formula provides better speech understandability and patient satisfaction than quick fitting.
