超声引导下双侧腹横肌平面阻滞联合地佐辛静脉泵注对肠癌术后镇痛效果患者疼痛度及预后分析

Analgesic effect,pain severity and outcome of ultrasound-guided bilateral transversus abdominis plane block combined with intravenous pump infusion of dezocine in patients after colon cancer surgery

目的 分析超声引导下给予肠癌手术患者双侧腹横肌平面阻滞(TAPB)联合地佐辛静脉泵注的镇痛效果及疼痛度、预后。方法 选取2020年5月至2021年5月到本院诊治的肠癌70例患者临床资料,按随机数表分2组,均采取开腹手术治疗,对照组35例采取地佐辛+舒芬太尼静脉泵注,研究组35例采取超声引导下TAPB联合地佐辛+舒芬太尼静脉泵注,观察2组镇痛效果及疼痛度、预后情况。结果 研究组下床活动的时间、肛门排气的恢复时间、住院的时间均比对照组短(P<0.05);术后4 h、8 h、16 h、24 h运动与静息状态研究组疼痛评分比对照组低,差异具有统计学意义(P<0.05);研究组不良反应发生率6%低于对照组(P<0.05)。结论 超声引导下给予肠癌手术患者TAPB联合地佐辛静脉泵注的镇痛效果较好,可有效改善患者的疼痛程度,减少不良反应,利于预后康复。

Objective To investigate the analgesic effect,pain severity and outcome of ultrasound-guided bilateral transversus abdominis plane block(TAPB)combined with intravenous pump infusion of dezocine in patients undergoing surgery for colon cancer.Methods The clinical data of 70 patients with colon cancer diag-nosed and treated in our hospital between May 2020 and May 2021 were collected and divided into two groups according to random number table.All patients underwent open abdominal surgery.Anesthesia was performed by intravenous pump infusion of dezocine and sufentanil in the control group(n=35),and by ultrasound-guided TAPB combined with intravenous pump infusion of dezocine and sufentanil in the study group(n=35).The anal-gesic effect,pain severity and patient outcome were compared between the two groups.Results The time to ambulation,time to anal exhaust,and length of hospital staly were shorter in the study group than those in the control group(P<0.05).The pain scores at rest and with activity in the study group at 4 h,8 h,16 h,and 24 h after surgery were lower than those in the control group,with statistically significant differences(P<0.05).The incidence of adverse reactions in the study group(6%)was lower than that in the control group(P<0.05).Conclusion Ultrasound-guided TAPB plus intravenous pump infusion of dezocine in patients undergoing colon cancer surgery leads to favorable analgesic effect,effective improvement in pain severity,and less adverse reac-tions,which is beneficial to patient prognosis and rehabilitation.
