HEART评分联合床边超声在高危胸痛患者诊断的临床应用研究

Clinical Application of HEART Score Combined with Bedside Ultrasound in The Diagnosis of High Risk Patients with Chest Pain

目的:探讨HEART评分联合床边超声在急诊急性高危胸痛患者的应用价值。方法:随机选择2021年9月至2022年9月到揭阳市人民医院急诊科就诊的急性胸痛患者,根据HEART评分选择高危胸痛患者,随机分成两组各102例,实验组予HEART评分联合床边超声结合临床表现进行诊断,对照组予HEART评分结合临床表现进行诊断,比较两组患者诊断的敏感度、特异度及准确性有无统计学差别。结果:实验组比对照组在急诊ACS(急性冠脉综合征)诊断上有更高的敏感度(P<0.05)、特异度(P<0.05)及准确性(P<0.05)。结论:在高危胸痛患者中使用床边超声联合HEART评分筛查急性ACS诊断上有更高的敏感度、特异度及准确性,更好地减少漏诊、误诊,值得推广。

Objective:To investigate the application value of HEART score combined with bedside ultrasound in emergency patients with acute high risk chest pain.Methods:Patients with acute chest pain who visited the emergency Department of Jieyang People's Hospital from September 2021 to September 2022 were randomly selected.Patients with high⁃risk chest pain were selected according to HEART score and randomly divided into two groups with 102 cases each.The experimental group was diagnosed with HEART score combined with bedside ultrasound and clinical manifestations.The control group was diagnosed with HEART score combined with clinical manifestations,and the comparison patients were clinically confirmed.The sensitivity,specificity and accuracy of diagnosis were calculated and compared between the two groups.Results:Compared with the clinical confirmation,the experimental group had higher sensitivity(P<0.05),specificity(P<0.05)and accuracy(P<0.05)in the clinical diagnosis of emergency ACS.Conclusion:Bedside ul⁃trasound combined with HEART score has higher sensitivity,specificity and accuracy in the diagnosis of acute ACS in high⁃risk patients with chest pain,and can better prevent missed diagnosis and misdiagnosis,which is worthy of promotion.
