基于影像学和组织学方法研究糖尿病大鼠腰椎间盘退变 被引量:1

Study on lumbar intervertebral disc degeneration in rats with diabetes mellitus based on imaging and histological methods

摘要 目的:探讨糖尿病大鼠模型腰椎间盘的变化及腰椎间盘退变的发生机制。方法:将50只雄性SD大鼠随机分为两组,其中30只作为模型组腹腔注射50 mg/kg链脲佐菌素建立糖尿病大鼠模型,另外20只作为正常对照组。在造模前及造模后第4、8、12周利用X线测量腰椎间隙高度,利用MRI的T2WI序列分析椎间盘信号强度;同期取大鼠腰椎间盘进行组织学评分,并分析影像学与组织学的关系。进一步检测血清中血脂、IL-6的变化。结果:链脲佐菌素腹腔注射成功建立糖尿病大鼠模型,成功率为60.0%。X线检测结果:造模后第8、12周糖尿病组大鼠腰椎间隙均减小,与对照组相比差异有统计学意义(P=0.002、P=0.001)。MRI检测结果:造模后第8、12周糖尿病组大鼠腰椎间盘退变等级均明显增加,与对照组相比差异有统计学意义(P=0.012、0.011)。造模后第8、12周糖尿病组大鼠组织评分显著增高,与对照组相比差异有统计学意义(P=0.000、0.000)。组织学评分与腰椎间隙高度呈负相关(r=-0.818,P=0.001),与腰椎间盘退变等级呈正相关(r=0.858,P=0.000)。糖尿病组大鼠血脂浓度增高,但与对照组相比差异无统计学意义(P>0.05)。造模后第8、12周糖尿病组大鼠血清中IL-6含量增高,与对照组相比差异有统计学意义(P=0.044、0.013)。结论:糖尿病大鼠模型发生腰椎间盘退变,且随着病程延长,退变等级增高,IL-6诱发的炎症反应可能是该变化的重要机制之一。 Objective:The purpose of this study was to investigate the changes and mechanisms of the lumbar intervertebral disc in a diabetic rat model.Methods:50 male SD rats were randomly divided into two groups.Thirty were used as model groups to establish diabetic rat models by intraperitoneal injection of streptozotocin at 50mg/kg.The height of the lumbar intervertebral disc space was measured by X-ray before and at 4,8 and 12 weeks after modelling.The signal intensity of the intervertebral disc was analyzed with T 2WI MRI.At the same time,the lumbar intervertebral discs were histologically scored.The relationship between imaging and histology was analyzed,and the changes in serum lipids and IL-6 were further detected.Results:Streptozotocin intraperitoneal injection successfully established the diabetic rat model;the success rate was 60.0%.X-ray exam results:lumbar vertebra space decreased in the diabetic group 8 and 12 weeks after modelling(P=0.002,P=0.001).MRI results:the grade of lumbar disc degeneration was significantly increased in the diabetic group at 8 and 12 weeks after modelling(P=0.012,P=0.011).The tissue score of the diabetic group was significantly increased at 8 and 12 weeks after modelling(P=0.000,P=0.000).The histological score was negatively correlated with lumbar space height(r=-0.818,P=0.001)and positively associated with lumbar disc degeneration(r=0.858,P=0.000).Serum lipid concentration increased in the diabetic group,but there was no statistical significance(P>0.05).Serum IL-6 content in the diabetic group increased 8 and 12 weeks after modelling(P=0.044,P=0.013).Conclusion:Lumbar intervertebral disc degeneration occurs in a diabetic rat model,and the grade of degeneration increases with the prolongation of the course of the disease,and IL-6-induced inflammatory response may be one of the crucial mechanisms of this change.
