凝血四项、D-二聚体与多发性骨髓瘤患者临床分期的相关性

Correlation between Four Items of Coagulation,D-Dimer and Clinical Stage of Patients with Multiple Myeloma

目的探讨凝血四项、D-二聚体与多发性骨髓瘤(MM)患者临床分期的相关性。方法选择2019年1月至2021年12月濮阳市人民医院收治的70例MM患者为研究对象,根据Durie-Salmon(DS)分期系统判定患者疾病临床分期,并分为Ⅰ期、Ⅱ期及Ⅲ期三组,统计患者基线资料、凝血四项[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)]、D-二聚体(D-D)值及其他实验室指标,采用双变量Kendall的tau-b(K)直线相关性分析凝血四项、D-D值与MM患者临床分期的相关性。结果根据DS分期法,70例MM患者中临床分期Ⅰ期31例(44.29%),Ⅱ期24例(34.29%),Ⅲ期15例(21.43%);Ⅰ期MM患者FIB、D-D、β2-MG、CRP值低于Ⅱ期、Ⅲ期MM患者,且Ⅱ期患者FIB、D-D、β2-MG、CRP值低于Ⅲ期MM患者,差异有统计学意义(P<0.05);双变量Kendall的tau-b(K)直线相关性分析显示,FIB、D-D、β2-MG、CRP值与MM患者临床分期呈正相关(r>0,P<0.05)。结论FIB、D-D与MM患者临床分期有关。

Objective To investigate the correlation between four items of coagulation,D-Dimer and clinical stage of patients with multiple myeloma(MM).Methods 70 patients with MM treated in Puyang People’s Hospital from January 2019 to December 2021 were selected as the research subjects.According to the Durie-Salmon(DS)staging system,the patients were divided into three groups:stageⅠ,stageⅡand stageⅢ.The baseline data,four items of coagulation[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)],D-Dimer(D-D)and other laboratory indexes of patients were counted.Bivariate Kendall’s tau-b(K)linear correlation was used to analyze the correlation between four coagulation items,D-D value and clinical stage of MM patients.Results According to the DS staging method,among the 70 patients with MM,there were 31 cases(44.29%)in stageⅠ,24 cases(34.29%)in stageⅡand 15 cases(21.43%)in stageⅢ;the values of FIB,D-D,β2-MG and CRP in patients with stageⅠMM were lower than those in patients with stageⅡandⅢMM,and the values of FIB,D-D,β2-MG and CRP in patients with stageⅡMM were lower than those in patients with stageⅢMM,the difference was statistically significant(P<0.05);bivariate Kendall’s tau-b(k)linear correlation analysis showed that the values of FIB,D-D,β2-MG and CRP were positively correlated with the clinical stage of MM patients(r>0,P<0.05).Conclusion FIB and D-D are related to the clinical stage of MM patients.
