单药或二联口服降糖药治疗血糖控制不佳的老年2型糖尿病患者加用达格列净的疗效及安全性

Efficacy and Safety of Single or Combined Oral Hypoglycemic Agents Plus Dagliazine in Elderly Patients with Type 2 Diabetes with Poor Glycemic Control

目的探讨单药或二联口服降糖药治疗血糖控制不佳的老年2型糖尿病患者加用达格列净的疗效及安全性。方法选取2018年2月至2021年9月在本院就诊的2型糖尿病患者82例,根据治疗方式的不同进行分组,将服用格列奈类药物治疗者纳入对照组,将服用格列奈类药物联合二甲双胍药物治疗者纳入观察组,均41例。统计两组患者BMI、FPG、2hPPG、HbA1c水平、胰岛功能、血脂、肾功能指标及不良反应发生率。结果治疗前,两组BMI、FPG、2hPPG、HbA1c水平,P>0.05,治疗后,两组BMI、FPG、2hPPG、HbA1c水平均下降,组间比较,观察组BMI、FPG、2hPPG、HbA1c水平均较对照组低,P<0.05。治疗前,两组FINS、2hPINS、HOMA-β水平,P>0.05,治疗后,两组FINS、2hPINS、HOMA-β水平均下降,组间比较,观察组FINS、2hPINS、HOMA-β水平均较对照组高,P<0.05。治疗前,两组TG、TC、Scr、BUN、ACR、eGFR水平,P>0.05,治疗后,两组TG、TC、Scr、BUN、ACR水平均下降,eGFR水平上升,组间比较,观察组TG、TC、Scr、BUN、ACR水平均较对照组高,eGFR水平较对照组低,P<0.05。两组不良反应发生率比较,P>0.05。结论单药或二联口服降糖药治疗血糖控制不佳的老年2型糖尿病患者加用达格列净,效果理想,可有效控制血糖水平,改善胰岛β细胞功能,安全性高,值得推广。

Objective To investigate the efficacy and safety of single or combined oral hypoglycemic agents plus dagliazine in the treatment of elderly patients with type 2 diabetes with poor blood glucose control.Groups were divided according to different treatment methods.Gleinide patients were included in the control group,and Gleinide patients combined with metformin were included in the observation group,with 41 cases in all.The levels of BMI,FPG,2hPPG,HbA1c,islet function,lipid and renal function and the incidence of adverse reactions in 2 groups were analyzed.Methods A total of 82 patients with type 2 diabetes treated in our hospital from February 2018 to September 2021 were selected.Results Before treatment,the levels of BMI,FPG,2hPPG and HbA1c in both groups,P>0.05;after treatment,the levels of BMI,FPG,2hPPG and HbA1c in both groups were decreased;compared between groups,the levels of BMI,FPG,2hPPG and HbA1c in the observation group were lower than those in the control group,P<0.05.Before treatment,FINS,2hPINS and HOMA-βlevels in both groups,P>0.05.After treatment,FINS,2hPINS and HOMA-βlevels in both groups were decreased.Compared between groups,FINS,2hPINS and HOMA-βlevels in the observation group were higher than those in the control group,P<0.05.Before treatment,TG,TC,Scr,BUN,ACR,eGFR levels in the two groups,P>0.05,after treatment,TG,TC,Scr,BUN,ACR levels in the two groups were decreased,eGFR levels were increased,comparison between groups,observation group TG,TC,Scr,BUN,ACR levels were higher than the control group.eGFR level was lower than that of control group(P<0.05).The incidence of adverse reactions was compared between the two groups,P>0.05.Conclusions Single drug or two oral hypoglycemic drugs in the treatment of elderly type 2 diabetes patients with poor blood glucose control plus dagliazine,the effect is ideal,can effectively control blood glucose level,improve the function of pancreaticβcells,high safety,worthy of promotion.
