摘要 目的 探究多模态磁共振(MRI)参数检测在前列腺疾病中应用价值。方法 对2019年8月至2022年8月医院收治82例前列腺疾病患者相关资料予以回顾性分析,患者均接受多模态MRI,随后接受病理检查确定疾病良恶性,分析多模态MRI在前列腺疾病诊断上价值;前列腺癌患者依据病理检查结果进行分级,分析多模态MRI参数与病理分级之间关系。结果 前列腺癌患者边界清晰比例显著低于前列腺良性病变,表观扩散系数(ADC)值与达峰时间(T_(max))低于前列腺良性病变,最快强化率(R_(max))、最大强化程度(SI_(max))高于前列腺良性病变(P<0.05);受试者工作特征曲线显示多模态MRI参数边界、ADC值、R_(max)、T_(max)、SI_(max)用于前列腺癌诊断曲线下面积(AUC)分别为0.686、0.788、0.782、0.811、0.602,各指标联合AUC为0.958;随着患者病理分级上升,前列腺癌患者ADC、T_(max)逐渐减小,而R_(max)及SI_(max)逐渐增大(P<0.05);Spearman相关性分析显示,ADC、T_(max)与前列腺癌病理分级负相关(r=-0.463,-0.376;P<0.05),R_(max)、SI_(max)与前列腺癌病理分级正相关(r=0.519,0.487;P<0.05)。结论 多模态MRI参数不仅有助于提高对于前列腺癌诊断价值,还与前列腺癌患者病理分级关系密切,可以为患者病情评估提供合适参考依据。 Objective To investigate the application value of multimodal magnetic resonance imaging(MRI)parameters in prostate diseases.Methods The relevant data of 82 patients with prostate diseases who were admitted to the hospital from August 2019 to August 2022 were retrospectively analyzed.All patients were examined with multimodal MRI,and then underwent pathological examination.The value of multimodal MRI in the diagnosis of prostate diseases,and the relationship between multimodal MRI parameters and pathological grading were analyzed.Results The proportion of lesions with clear boundaries,apparent diffusion coefficient(ADC)value and time to peak(T_(max))in prostate cancer patients were significantly lower/shorter than those in patients with benign prostate lesions.The _(max)imum enhancement rate(R_(max))and _(max)imum signal intensity(SI_(max))were higher than those in patients with benign prostate lesions(P<0.05).The receiver operating characteristic curve showed that the area under the curve(AUC)values of multimodal MRI parameters(boundary,ADC value,R_(max),T_(max),and SI_(max))to diagnose prostate cancer were 0.686,0.788,0.782,0.811,and 0.602,respectively.The AUC of combined diagnosis with these indicators was 0.958.With the increase of pathological grade,ADC and T_(max) of prostate cancer patients decreased,while R_(max) and SI_(max) increased(P<0.05).Spearman correlation analysis showed that ADC and T_(max) were negatively correlated with the pathological grade of prostate cancer(r=-0.463,-0.376,P<0.05),while R_(max) and SI_(max) were positively correlated with the pathological grade of prostate cancer(r=0.519,0.487,P<0.05).Conclusion Multimodal MRI parameters can help to improve the diagnosis of prostate cancer,and are closely related to the pathological grade of prostate cancer,which provides reference for condition evaluation.
