摘要
目的 探讨胰岛素泵在儿童糖尿病酮症酸中毒治疗中的疗效。方法 选择大冶市人民医院2020年5月—2022年5月收治的60例儿童糖尿病酮症酸中毒患儿,按随机数表法分成两组,各30例。对照组给予胰岛素静脉滴注治疗,观察组给予胰岛素泵治疗,对比两组治疗效果。结果 两组治疗前和治疗后的空腹血糖、餐后2 h血糖值相比,差异无统计学意义(P>0.05);观察组尿酮体转阴、血糖达标、p H恢复和住院时间较对照组短,胰岛素用量、日均胰岛素用量、每人每天低血糖发生次数比对照组少,差异有统计学意义(P<0.05);治疗后,观察组糖化血红蛋白(Hb Alc)、阴离子间隙(AG)值低于对照组,二氧化碳结合力(CO_(2)-CP)、C肽值高于对照组,差异有统计学意义(P<0.05)。结论 胰岛素泵治疗儿童糖尿病酮症酸中毒能缩短血糖达标、尿酮体转阴、p H恢复及住院时间,减少患儿胰岛素用量与低血糖发生次数,临床疗效显著。
Objective To investigate the effect of insulin pump in the treatment of children with diabetic ketoacidosis.Methods Sixty children with diabetic ketoacidosis from Daye People´s Hospital from May 2020 to May 2022 were selected and divided into two groups according to random number table method,with thirty cases in each group.The control group was given insulin intravenous drip treatment,and the observation group was given insulin pump treatment.The therapeutic effect of the two groups was compared.Results There were no statistically significant difference in the fasting blood glucose and the 2-hour postprandial blood glucose after meal between the two groups before and after treatment(P>0.05).Urine ketone body turning negative,blood glucose reaching standard,pH recovery and hospital stay in the observation group were shorter than those in the control group,insulin consumption,daily insulin consumption and the occurrence times of hypoglycemia per person per day were lower than those in the control group,the difference was statistically significant(P<0.05).After treatment,the glycosylated hemoglobin(HbAlc)and anion gap(AG)values in observation group were lower than those in control group,while the carbon dioxide binding force(CO_(2)-CP)and C-peptide values in observation group were higher than those in control group,the difference was statistically significant(P<0.05).Conclusion The treatment of children with diabetic ketoacidosis with insulin pump can shorten the time of blood glucose standard,urine ketone body negative,pH recovery and hospital,and reduce the dosage of insulin and the frequency of hypoglycemia in children.The clinical effect is significant.
