纤维支气管镜肺泡灌洗在老年重症肺炎中的应用效果分析

Application Effect Analysis of Bronchoscopic Alveolar Lavage in Senile Severe Pneumonia

目的:探讨纤维支气管镜肺泡灌洗治疗老年重症肺炎患者的临床疗效。方法:选取2020年2月—2022年2月黔东南州中医医院重症科收治的老年重症肺炎患者88例为观察对象,根据随机数字表法分为对照组及研究组,各44例。对照组给予常规治疗,研究组在对照组基础上给予纤维支气管镜肺泡灌洗治疗。比较两组患者治疗效果、相关时间指标、呼吸功能、炎性因子水平。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);研究组咳嗽咳痰消失、体温恢复正常、肺部啰音消失、住院时间均短于对照组,差异有统计学意义(P<0.05);治疗后,两组患者二氧化碳分压(PaCO_(2))水平均降低,血氧饱和度(PaO_(2))、动脉血氧饱和度(SaO_(2))水平均升高,研究组PaCO_(2)水平低于对照组,PaO_(2)、SaO_(2)水平高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者C反应蛋白、降钙素原水平均降低,研究组低于对照组,差异有统计学意义(P<0.05)。结论:在治疗老年重症肺炎患者中应用纤维支气管镜肺泡灌洗效果显著,可有效改善患者临床症状,恢复呼吸功能,降低炎性因子水平,缩短症状改善时间。

Objective:To investigate the clinical effect of bronchoscopic alveolar lavage on senile patients with severe pneumonia.Methods:A total of eighty-eight elderly patients with severe pneumonia admitted to the Department of Intensive Care of Qiandongnan Prefecture Hospital of Traditional Chinese Medicine from February 2020 to February 2022 were selected as the observation objects,and were divided into control group and study group according to random number table method,with forty-four cases in each group.The control group received conventional treatment,and the study group received bronchoscopic alveolar lavage on the basis of the control group.The therapeutic effect,relevant time indexes,respiratory function and inflammatory factors were compared between the two groups.Results:The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The cough and phlegm disappeared,body temperature returned to normal,lung rales disappeared and hospital stay in the study group were shorter than those in the control group,the difference was statistically significant(P<0.05).After treatment,the level of partial carbon dioxide pressure(PaCO_(2))decreased,and the level of blood oxygen saturation(PaO_(2))and arterial oxygen saturation(SaO_(2))increased in both groups.PaCO_(2) level in the study group was lower than that in the control group,while PaO_(2) and SaO_(2) levels were higher than that in the control group,with statistical significance(P<0.05).After treatment,C-reactive protein and calcitonin raw water in 2 groups were decreased on average,and the study group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:In the treatment of elderly patients with severe pneumonia,the application of bronchoscopic alveolar lavage has significant effects,which can effectively improve the clinical symptoms of patients,restore respiratory function,reduce the level of inflammatory factors,and shorten the time of symptom improvement.
