高危型人乳头状瘤病毒分型检测在早期宫颈癌患者中的诊断价值分析

Analysis of the Diagnostic Value of High-risk Human Papillomavirus Typing in Patients with Early Cervical Cancer

目的:探讨高危型人乳头状瘤病毒(HPV)分型检测在早期宫颈癌患者中的诊断价值与诊断效能。方法:选取2020年2月—2022年2月临沧市人民医院就诊的疑似早期宫颈癌患者327例为观察对象。患者均收集宫颈分泌物接受高危型HPV分型检测,以病理活检结果为评价依据,计算高危型HPV分型检测在早期宫颈癌中的诊断效能。结果:327例病理活检结果显示,正常/宫颈炎44.65%(146/327),宫颈上皮内瘤变(CIN)Ⅰ17.13%(56/327),CINⅡ21.41%(70/327),CINⅢ11.62%(38/327),鳞状细胞癌4.78%(17/365),病理活检阳性率为55.35%(181/327)。高危型HPV分型检测结果和病理检查的符合率为85.02%(278/327),诊断灵敏度88.40%(160/181),特异度81.51%(119/146)。结论:高危型HPV分型检测在早期宫颈癌患者中的诊断价值显著,诊断灵敏度、特异度较高,有助于提升诊断准确率,为临床后续工作提供可靠依据。

Objective:To investigate the diagnostic value and diagnostic efficiency of high-risk human papillomavirus(HPV)typing in patients with early cervical cancer.Methods:From February 2020 to February 2022,327 patients with suspected early cervical cancer who were treated in Lincang People's Hospital were selected as observation objects.Cervical secretions were collected from all patients for high-risk HPV typing detection,and the pathological biopsy results were used as the evaluation basis to calculate the diagnostic efficiency of high-risk HPV typing detection in early cervical cancer.Results:The pathological biopsy results of 327 cases showed that normal/cervicitis was 44.65%(146/327).Cervical intraepithelial neoplasia(CIN)Ⅰ17.13%(56/327),CINⅡ21.41%(70/327),CINⅢ11.62%(38/327),squamous cell carcinoma 4.78%(17/365).The positive rate of pathological biopsy was 55.35%(181/327).The coincidence rate of high-risk HPV typing test results and pathological examination was 85.02%(278/327).The diagnostic sensitivity was 88.40%(160/181),and the specificity was 81.51%(119/146).Conclusion:High-risk HPV typing detection has significant diagnostic value in patients with early cervical cancer,with high diagnostic sensitivity and specificity,which is helpful to improve the diagnostic accuracy and provide a reliable basis for clinical follow-up work.
