宫颈环形电切术与宫颈冷刀锥切术对高级别宫颈鳞状上皮内病变患者并发症及预后的影响

Effect of cervical ring electrotomy and cervical cold knife conectomy on complications and prognosis of patients with high-grade cervical squamous intraepithelial lesions

目的探究宫颈环形电切术与宫颈冷刀锥切术对高级别宫颈鳞状上皮内病变患者并发症及预后的影响。方法选取本院收治的50例高级别宫颈鳞状上皮内病变患者,依据随机数字表法将其分为对照组与研究组,对照组实施宫颈环形电切术,研究组实施宫颈冷刀锥切术。比较两组患者治疗效果、复发率、并发症、预后以及手术时间、术中出血量与创面愈合时间。结果与对照组相比较,研究组患者治疗总有效率更高,并发症发生率、早期流产率、晚期流产率、复发率均更低;研究组患者手术时间延长,术中出血量更多,但创面愈合时间均更短,差异具有统计学意义(P<0.05)。结论两种手术对高级别宫颈鳞状上皮内病变患者治疗均有一定效果,但宫颈冷刀锥切术可以缩短创面愈合时间,减少术后并发症与复发率,但手术时间相对偏久,术中出血量相对偏多。临床可根据患者病情及意愿,选择最佳手术方式。

Objective To investigate the effects of cervical ring electrotomy and cervical cold knife conectomy on the complications and prognosis of patients with high-grade cervical squamous intraepithelial lesions.Methods Fifty patients with high-grade cervical squamous intraepithelial lesions admitted to our hospital were selected and divided into the control group and the study group according to random number table method.The control group underwent cervical ring electrotomy,and the study group underwent cervical cold knife conectomy.The therapeutic effect,recurrence rate,complications,prognosis,operative time,intraoperative blood loss and wound healing time were compared between the two groups.Results Compared with the control group,the total effective rate of treatment in the study group was higher,the complication rate,early abortion rate,late abortion rate and recurrence rate were lower,the patients in the study group had longer operation time,more blood flow during operation,but shorter wound healing time,the difference was statistically significant(P<0.05).Conclusion Both of the two operations have certain effects on the treatment of high-grade cervical squamous intraepithelial lesions,but the cold knife cone cut method can shorten the wound healing time,reduce the postoperative complications and the recurrence rate,but relatively more operative time and intraoperative blood loss.We can choose the best surgical procedure according to the patients'condition and willingness.
