经阴道超声对不同年龄卵巢多囊样改变的诊断价值分析

The diagnostic value of transvaginal ultrasonography for po1ycystic ovarian morphology of ovaries at different ages

目的探讨经阴道超声在不同年龄卵巢多囊样改变(PCOM)诊断中的应用价值。方法回顾性选取68例多囊卵巢综合征(PCOS)患者,纳入PCOS组,同时选择68例健康体检女性,纳入对照组。两组受检者均接受经阴道超声检查,比较两组不同年龄受检者的卵巢体积、卵泡数,分析卵巢体积、卵泡数诊断不同年龄段PCOM的截断值。结果PCOS组不同年龄段受检者的卵巢体积均大于对照组,卵泡数均多于对照组,差异具有统计学意义(P<0.05)。经ROC曲线分析,不同年龄段受检者经卵巢体积诊断PCOM最佳截断值分别为13 mL、10 mL、9 mL、9 mL、9 mL,曲线下面积分别为0.763、0.733、0.822、0.870、0.759。卵泡数诊断PCOM最佳截断值分别为13个、13个、9个、9个、9个,曲线下面积分别为0.848、0.855、0.891、0.754、0.889。结论经阴道超声诊断PCOM具有较高的应用价值,而考虑到不同年龄段人群的卵巢形态改变,应对诊断PCOM截断值进行调整,为下一步治疗提供依据。

Objective To investigate the application value of transvaginal ultrasound in the diagnosis of po1ycystic ovarian morphology(PCOM)at different ages.Methods 68 patients with polycystic ovary syndrome(PCOS)were retrospectively selected and included in the PCOS group,while 68 healthy women were selected and included in the control group.The subjects in both groups received transvaginal ultrasound examination.The ovarian volume and follicle number of subjects in the two groups at different ages were compared,and the truncated value of PCOM at different ages was analyzed.Results The ovarian volume and follicle number in the PCOS group at different ages were higher than those in the control group,and the difference was statistically significant(P<0.05).According to ROC curve analysis,the optimal cut-off values of PCOM for ovarian volume diagnosis in different age groups were 13 mL,10 mL,9 mL,9 mL and 9 mL respectively,and the areas under the curve were 0.763,0.733,0.822,0.870 and 0.759 respectively.The optimal truncation values of PCOM in the diagnosis of follicle number were 13,13,9,9,9,and the area under the curve were 0.848,0.855,0.891,0.754 and 0.889,respectively.Conclusion Transvaginal ultrasonography has high application value in the diagnosis of PCOM.Considering the changes of ovarian morphology in different age groups,the truncation value of PCOM should be adjusted to provide basis for further diagnosis and treatment.
