腹腔镜卵巢囊肿剔除术治疗卵巢囊肿的临床效果及其对卵巢功能的影响

Clinical Effect of Laparoscopic Ovarian Cystectomy in the Treatment of Ovarian Cysts and its Effect on Ovarian Function

目的探究腹腔镜卵巢囊肿剔除术(LOC)治疗卵巢囊肿的临床效果及其对卵巢功能的影响。方法选取2020年6月-2022年1月天津市武清区人民医院收治的87例卵巢囊肿患者作为研究对象,采用随机数字表法分为对照组(n=43)和观察组(n=44)。对照组行开腹卵巢囊肿剔除术治疗,观察组行LOC治疗,比较两组临床疗效、围术期指标(术中出血量、手术时间、胃肠道功能恢复时间、术后住院时间)、卵巢功能[雌二醇(E_(2))、促卵泡激素(FSH)、黄体生成素(LH)]、卵巢储备功能(卵巢体积、卵泡数量)、并发症发生率。结果两组治疗总有效率比较,差异无统计学意义(P>0.05);观察组术中出血量少于对照组,且手术时间、胃肠道功能恢复时间、术后住院时间均短于对照组(P<0.05);两组术后FSH、LH水平高于术前,E_(2)水平低于术前,但观察组FSH、LH低于对照组,E_(2)高于对照组(P<0.05);两组术后卵泡数量、卵巢体积低于术前,但观察组高于对照组(P<0.05);观察组并发症发生率为4.55%,低于对照组的16.28%(P<0.05)。结论LOC治疗卵巢囊肿疗效确切,其出血少、恢复快,可减少手术对卵巢功能的影响,缓解卵巢储备功能损害,且并发症风险低。

Objective To explore the clinical effect of laparoscopic ovarian cystectomy(LOC)in the treatment of ovarian cysts and its effect on ovarian function.Methods A total of 87 patients with ovarian cysts admitted to Tianjin Wuqing District People's Hospital from June 2020 to January 2022 were selected as the research objects,and they were divided into control group(n=43)and observation group(n=44)by random number table method.The control group was treated with open ovarian cystectomy,and the observation group was treated with LOC.The clinical efficacy,perioperative indicators(intraoperative blood loss,operation time,gastrointestinal function recovery time,postoperative hospital stay),ovarian function[estradiol(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH)],ovarian reserve function(ovarian volume,number of follicles),and complication rate were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups(P>0.05).The intraoperative blood loss in the observation group was less than that in the control group,and the operation time,gastrointestinal function recovery time and postoperative hospitalization time were shorter than those in the control group(P<0.05).The levels of FSH and LH in the two groups were higher than those before operation,and the level of E_(2)was lower than that before operation,but the levels of FSH and LH in the observation group were lower than those in the control group,and the level of E_(2)was higher than that in the control group(P<0.05).The number of follicles and ovarian volume in the two groups were lower than those before operation,but the observation group was higher than the control group(P<0.05).The incidence of complications in the observation group was 4.55%,which was lower than 16.28%in the control group(P<0.05).Conclusion LOC is effective in the treatment of ovarian cysts,with less bleeding and faster recovery.Meanwhile,it can reduce the impact of surgery on ovarian function,alleviate the damage of ovarian reserve function,and has a low risk of complications.
