超声在克罗恩病诊断中的应用价值

Application value of ultrasound in the diagnosis of Crohn's disease

克罗恩病(Crohn’s disease,CD)是一种慢性炎症性肠病,可导致胃肠道进行性损伤和残疾,有终生复发的倾向,病变部位呈节段性或者跳跃式分布,多见于回肠末段和邻近结肠,从口腔至肛门各段消化道均可受累。本病在欧美多见,近年来我国CD发病率逐渐升高。CD的诊断主要从实验室检查、影像学检查以及肠镜检查方面进行。其中影像学检查至关重要,包括腹部超声、CT及磁共振肠道成像(CT/MR、CTE/MRE)。CT及磁共振肠道成像由于具有较强的辐射或价格昂贵等局限,超声在诊断CD中的表现越来越突出。为了全面地了解超声在诊断及治疗CD中的应用价值,本文就超声在评估CD的新近研究做一综述。

Crohn's disease(CD)is a chronic inflammatory bowel disease that can lead to progressive bowel injury and disability,with a tendency to relapse for life,more common in the terminal ileum and adjacent colon,but can affect all segments of the digestive tract from the mouth to the anus,with a segmental or skipping distribution.The disease is more common in Europe and the United States,and the incidence of CD in China has gradually increased in recent years.The diagnosis of CD is mainly performed on laboratory tests,imaging tests,and colonoscopy.Imaging studies,including abdominal ultrasound,CT and magnetic resonance imaging(CT/MR,CTE/MRE)of the bowel are essential.Due to the limitations of CT and magnetic resonance imaging of the bowel,ultrasound is becoming more and more prominent in the diagnostic CD.In order to comprehensively understand the application value of ultrasound in the diagnosis and treatment of CD,this article reviews the recent research on ultrasound in the evaluation of CD.
