摘要
目的研究腰腿痛宁胶囊对木瓜蛋白酶诱导的兔膝骨性关节炎模型炎症因子的调控机制。方法20只实验兔随机分为空白组、模型组、仙灵骨葆组和腰腿痛宁组,各5只。各组左膝关节腔注射木瓜蛋白酶混合液0.5 mL·kg^(-1),并驱赶运动14 d,然后分别采用对应药物给予治疗28 d。同时,各组在治疗前(造模成功后)和治疗后用量角器测量左膝关节活动角度。实验结束后,拍摄X线片观察各组左膝关节损伤情况;HE染色法观察各组关节软骨病理改变,进行Mankin评分;ELISA法测定各组血清和关节液中IL-1β、IL-6、TNF-α的水平。结果治疗前,与空白组比较,其他各组左膝关节活动度明显降低(P<0.05),X线片显示左膝关节间隙明显变窄、骨赘形成;治疗后,与空白组比较,模型组左膝关节软骨损伤程度及Mankin评分明显增高(P<0.05),炎症因子水平均显著升高(P<0.05);治疗后,与模型组比较,仙灵骨葆组和腰腿痛宁组左膝关节活动度均改善(P<0.05),左膝关节软骨损伤程度及Mankin评分明显降低(P<0.05),炎症因子水平水平显著降低(P<0.05)。结论腰腿痛宁胶囊能降低兔早期膝骨关节炎血清和关节液中IL-1β、IL-6、TNF-α的水平,提示腰腿痛宁胶囊可通过抑制炎症反应延缓膝骨关节炎病理发展。
Objective To investigate the regulatory mechanism of Yaotui Tongning Capsules on inflammatory factors in rabbit knee osteoarthritis model induced by papain.Methods 20 Rabbits were randomly divided into the blank group(n=5),the model group(n=5),Xianlinggubao group(n=5)and Yaotui Tongning group(n=5).Rabbit left knee joint cavity of each group was injected with 0.5 mL·kg^(-1)of papain mixture and driven for 14 d,and then rabbit knee osteoarthritis was were treated with corresponding drugs for 28 days.The angle of movement of the left knee joint of each group was measured with a goniometer before(after successful molding)and after treatment in each group.At the end of the trial,X-ray fi lms were taken to observe damage to the left knee joint of each group;HE staining was used to observe pathological changes in the articular of each group,and the Mankin's score was used;the levels of IL-1β,IL-6 and TNF-αin the serum and joint fluid of each group were measured by ELISA.Results Compared with the blank group,the mobility of the left knee joint of the other groups was signifi cantly reduced(P<0.05),and the X-ray fi lms showed that the left knee joint space was signifi cantly narrowed and osteophytes were formed;After treatment,compared with the blank group,the degree of cartilage damage and Mankin's score were signifi cantly increased(P<0.05),and the levels of infl ammatory factors in the model group were all signifi cantly increased(P<0.05).After treatment,compared with the model group,in Xianling Gubao group and Yaotui Tongning group the mobility of the left knee joint improved(P<0.05),the degree of cartilage damage and Mankin's score decreased signifi cantly(P<0.05),and the levels of infl ammatory factors decreased signifi cantly(P<0.05).Conclusion Yaotui Tongning capsules can reduce the levels of IL-1β,IL-6 and TNF-αin the serum and joint fl uid of rabbits with early knee osteoarthritis,suggesting that Yaotui Tongning capsules can delay the pathological development of knee osteoarthritis by inhibiting the infl ammatory response.
