常见血炎症标志物在慢性阻塞性肺疾病急性加重期中的预测价值

Predictive value of common blood inflammatory markers and acute exacerbations of chronic obstructive pulmonary disease

慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)正逐渐成为全球第三位主要死因,其中慢性阻塞性肺疾病急性加重期(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)是造成COPD患者高死亡率以及巨大医疗成本的主要原因。虽然近年临床上涌现出多种预测COPD患者病情严重程度的评分系统,但目前对COPD病情评估及诊治仍存在不足。本文就常见血炎症标志物在AECOPD诊治进展方面进行综述,发现通过检测炎症标志物,可鉴别呼吸道感染病原菌,有利于指导AECOPD治疗;进一步分析炎症标志物与疾病严重程度相关性,提出新的病情评估指标,有利于早期识别重症AECOPD患者,及早进行干预,从而降低AECOPD病死率,改善患者病情预后,有望联合炎症标志物建立相关AECOPD预测模型,为寻找新的诊断标志物提供理论依据。

Chronic obstructive pulmonary disease(COPD)is becoming the third leading cause of death worldwide,with acute exacerbation of chronic obstructive pulmonary.The acute exacerbation of chronic obstructive pulmonary disease(AECOPD)is responsible for the high mortality rate of COPD patients and the huge medical costs.Although several scoring systems have emerged in recent years to predict the severity of COPD patients,there are still deficiencies in the assessment and management of COPD disease.This paper will review the progress of common blood inflammatory markers in the diagnosis and treatment of AECOPD.It is found that inflammatory markers can identify the pathogenic bacteria of respiratory tract infection,which is helpful to guide the treatment of AECOPD.It will also further analyze the correlation between inflammatory markers and disease severity,and propose a new index for disease assessment,which is helpful to identify patients with severe AECOPD earlier and intervene earlier,thus reducing the mortality of AECOPD and improving the prognosis of patients.It is expected to establish a relevant AECOPD prediction model in conjunction with inflammatory markers,and to provide a theoretical basis for finding new diagnostic markers.
