急性等容血液稀释自体输血技术的临床应用研究进展

Research progresses on clinical application of acute normovolemic hemodilution autologous blood transfusion

血液资源紧张和异体输血风险一直是外科手术的难题,新冠疫情期间更是加重了血液短缺,因用血安全无法保证,导致部分患者手术延期。随着围术期患者血液管理的应用及发展,急性等容血液稀释(Acute normovolaemichemodilution,ANH)已成为手术中减少同种异体输血的重要策略之一,缓解了临床血液供需矛盾。本文就近年来急性等容稀释性自体输血技术的相关生理变化、实施要点及在不同手术中的临床应用进行综述,以期为合理使用ANH技术提供指导。

The shortage of blood resources and the risk of allogeneic transfusion has always been a difficult problem for surgical operations,and the shortage of blood had been aggravated during the pandemic of COVID-19.As blood safety cannot be guaranteed,surgical treatment of some patients had been delayed.With the development ofprioperative blood management and a series of measures proposed,easing the conflict between clinical blood supply and demand.Acute normovolaemic hemodilution(ANH)has become an important strategy for reducing allogeneic transfusion in surgery.This review summarizes the physiological changes,volume management and application of ANH in different operations in recent years,tryingto provide some guidance for ANH's rational use.
