输卵管妊娠合并子宫血管周上皮样细胞肿瘤1例

One case of tubal pregnancy complicated with uterine perivascular epithelioid cell tumor

目的 总结1例无症状子宫血管周上皮样细胞肿瘤(PEComa)的相关诊疗要点。方法 回顾性分析2017年12月甘肃省人民医院妇科收治的1例输卵管妊娠合并子宫PEComa病人的临床资料,并复习国内外相关文献。结果 该例因异位妊娠入院,经彩超检查发现合并子宫前壁肿物,入院诊断:异位妊娠;子宫平滑肌瘤。行手术治疗,术后病理结果为子宫血管周上皮样细胞肿瘤。术后未行进一步治疗,随访至今未见复发及转移,预后良好。结论 子宫血管周上皮样细胞肿瘤病例少见,临床表现无特异性,影像学检查容易误诊,诊断主要依靠组织病理和免疫组织化学检查,手术是主要治疗方式,术后严密随访是关键。

Objective To summarize the related diagnosis and treatment points of an asymptomatic case of perivascular epithelioid cell tumor(PEComa)of the uterus.Methods The clinical data of 1 patient with tubal pregnancy complicated with uterine PEComa admitted to the Department of Gynecology of Gansu Provincial People's Hospital in December 2017 were retrospectively analyzed,and relevant domestic and foreign literature was reviewed.Results The patient was admitted to the hospital for and was found to have a combined anterior uterine wall mass on ultrasound.The admission diagnosis was ectopic pregnancy and uterine leiomyoma.The patient underwent surgery and postoperative pathological findings of the fibroid were perivascular epithelioid tumor of the uterus.No further treatment was performed after the operation,and no recurrence or metastasis was found in the follow-up thus far,with a good prognosis.Conclusions Perivascular epithelioid tumors of the uterus are rare,and the clinical manifestations are nonspecific.The diagnosis mainly relies on histopathological and immunohistochemical examinations.
