探析腹腔镜辅助胃癌根治术应用于进展期胃癌的效果观察

Effect Observation and Analysis of Laparoscopic Assisted Radical Gastrectomy for Advanced Gastric Cancer

目的 观察腹腔镜辅助下胃癌根治术在进展期胃癌治疗中的临床效果。方法 以本院2019年9月-2020年10月收治的进展期胃癌患者86例为研究对象,以随机方式分为对照组与实验组,每组各43例,所有患者均接受常规的化疗干预,在此基础上,对照组予以开腹胃癌根治术治疗,实验组则以腹腔镜辅助胃癌根治术治疗,观察两组治疗的效果、各项手术指标情况、生活质量情况及不良反应情况。结果 两组切口长度、术中出血量等指标相比,差别有统计学意义(P<0.05);手术时间及淋巴结清扫数目两组相比,差异无统计学意义(P>0.05);实验组的术后肛门排气、恢复进食、下床活动及住院时间均比对照组短,差别有统计学意义(P<0.05);实验组生活质量改善总有效率优于实验组,差别有统计学意义(P<0.05);两组切口感染、吻合口瘘、肺部感染、肠梗阻等并发症的总发生率对照组高于实验组,差别有统计学意义(P<0.05)。结论 腹腔镜辅助胃癌根治术在进展期胃癌的治疗中能够显著提高治疗效果,改善患者的生活质量,减少不良反应,推广价值高。

Objective To observe clinical effect of laparoscopic assisted radical gastrectomy in treatment of advanced gastric cancer.Methods The paper chose 86 patients with advanced gastric cancer admitted to our hospital from September 2019 to October 2020,and divided them into control group and an experimental group randomly,with 43 cases in each group.All patients were treated with routine chemotherapy intervention.On the basis,control group was treated with open radical gastrectomy for gastric cancer,while experimental group with laparoscopically assisted radical gastrectomy.Curative effect and various surgical indicators,life quality and adverse reactions between two groups were observed.Results There was statistically significant difference between two groups in terms of incision length and intraoperative bleeding(P<0.05).There was no significant difference between two groups in surgical time and lymph node dissection number(P>0.05).Postoperative anal discharge,resumption of eating,ambulation,and hospital stay in experimental group was significantly shorter than control group(P<0.05).Total effective rate of improving life quality in control group was 76.74%,that of experimental group was 90.70%,with statistically significant difference(P<0.05).Total incidence of complications including incision infection,anastomotic leakage,pulmonary infection,and intestinal obstruction in control group was higher than experimental group,with statistically significant difference(P<0.05).Conclusion Laparoscopic assisted radical gastrectomy can significantly improve treatment effect,life quality,reduce adverse reactions of advanced gastric cancer patients,which has high promotion value.
