无创呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭的价值

Value of Noninvasive Ventilation in Treatment of Chronic Obstructive Pulmonary Disease with Respiratory Failure

目的 探究慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者实施无创呼吸机治疗的应用价值。方法随机抽选2020年1月-2022年1月盱眙县人民医院收治的慢阻肺合并呼吸衰竭患者120例进行研究,根据红白小球抽签结果分作观察组(红球,60例)与对照组(白球,60例),对照组实施常规方案治疗,观察组在其基础上联合无创呼吸机治疗,观察各组患者治疗效果,检测对比两组患者治疗前后肺功能(FEV1、FVC、FEV1/FVC)及血气指标(PaO_(2)、PaCO_(2)、SaO_(2))变化趋势,观察统计各组患者并发症情况。结果 观察组慢阻肺合并呼吸衰竭患者治疗后咳嗽、咳痰、气喘等症状评分明显低于对照组(P<0.05)。入院时检测两组患者肺功能及血气功能指标无统计学差异(P>0.05),治疗一段时间后患者各项指标均有明显改善,观察组患者肺功能检测结果均高于对照组,同时血气指标中PaO_(2)、SaO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05)。观察组患者出现并发症的总概率低于对照组(P<0.05)。结论 慢性阻塞性肺疾病合并呼吸衰竭患者实施无创呼吸机治疗具有确切疗效,能够有效改善患者肺功能,调节血气指标,减少患者并发症,倡导运用推广。

Objective To explore application value of noninvasive ventilator therapy in treatment fo patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Methods The paper carried oun randomized study on 120 COPD patients complicated with respiratory failure admitted to Xuyi County People's Hospital from January 2020 to January 2022,and divided them into observation group(60 patients with red and white globules)and control group(60 patients with white globules)based on the results of red and white globules drawn.Control group was treated with routine regimen,and observation group with non-invasive ventilation on the basis of routine scheme.Curative effect of each group was observed,changes in pulmonary function(FEV1,FVC,FEV1/FVC)and blood gas indicators(PaO_(2),PaCO_(2),SaO_(2))before and after treatment were detected and compared between two groups,complications of each group were observed and counted.Results Symptoms scores including cough,expectoration,and asthma in observation group after treatment were significantly lower than control group(P<0.05).Upon admission,there was no statistically significant difference in pulmonary function and blood gas function indicators between two groups(P>0.05).After a period of treatment,various indicators of patients were significantly improved.Pulmonary function test results of observation group were higher than control group,while blood gas indicators PaO_(2) and SaO_(2) were higher than control group,while PaCO_(2) was lower than control group(P<0.05).Total complications incidecne in observation group was lower than control group(P<0.05).Conclusion Non invasive ventilator treatment can achieve definite effect for patients with chronic obstructive pulmonary disease and respiratory failure,can improve lung function,regulate blood gas indicators,reduce patient complications effectively,which is worthy of application and promotion.
