血清PCT、hs-CRP、IFN-γ、IL-10水平和产妇产褥感染的相关性

Correlation between serum PCT,hs-CRP,IFN-γ,IL-10 levels and maternal puerperal infection

目的 研究产妇产褥感染与血清降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)、干扰素-γ(IFN-γ)、白细胞介素10(IL-10)水平的相关性。方法 选择2018年5月至2022年10月间郑州大学第二附属医院收治的108例发生产褥感染的产妇及95例未发生产褥感染的产妇,并设为感染组及未感染组。对比两组孕产妇临床资料、分娩前后的血清PCT、hs-CRP、IFN-γ、IL-10水平情况,统计产褥感染部位构成比,并采用Spearman法统计上述因子与产褥感染的相关性。结果 两组孕周、产次、合并高血压、糖尿病等资料比较差异无统计学意义(P>0.05);与分娩前比较,分娩24 h后两组PCT、hs-CRP、IL-10水平上升,IFN-γ水平下降,其中感染组各因子水平变化最明显,差异有统计学意义(P<0.05);采用Spearman法分析结果显示血清PCT、hs-CRP、IL-10水平与产褥感染呈正相关,而血清IFN-γ与产褥感染呈负相关(P<0.05);108例产褥感染患者中子宫内膜、盆腔腹膜、外阴道及阴道感染患者居多。结论 血清PCT、hs-CRP、IFN-γ、IL-10与产妇产褥感染存在相关性,可作为产褥感染评估的重要指标。

Objective To study the correlation between maternal puerperal infection and serum procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP),interferon-γ(IFN-γ),interleukin-10(IL-10)levels.Methods 108 puerperal women with puerperal infection and 95 puerperal women without puerperal infection admitted to the Second Affiliated Hospital of Zhengzhou University from May 2018 to October 2022were selected and divided into the infection group and non-infection group.Comparison of clinical data of pregnant women,serum PCT,hs-CRP,IFN-γ and IL-10 levels before and after delivery between the two were compared,the composition ratio of puerperal infection site was calculated,and the correlation between the above factors and puerperal infection was calculated by Spearman method.Results There was no significant difference between the two groups in terms of gestational age,parity,hypertension,and diabetes and other data(P>0.05).Compared with before treatment,the levels of PCT,hs-CRP,and IL-10 in the two groups increased24 hours after delivery,while the levels of IFN-γ decreased,and the levels of each factor in the infection group changed most significantly,and the difference was statistically significant(P<0.05).The Spearman method analysis showed that serum PCT,hs-CRP,IL-10 levels were positively correlated with puerperal infection,while serum IFN-γ was positively correlated with puerperal infection(P<0.05).Among the 108 cases of puerperal infection,endometrium,pelvic peritoneum,vulva and vaginal infection were the majority.Conclusion Serum PCT,hs-CRP,IFN-γ,IL-10 are correlated with puerperal infection,and can be used as important indicators for the evaluation of puerperal infection.
