疼痛专科护士主导多学科协作模式对居家癌痛病人生活质量的影响

Impact of multidisciplinary collaboration model led by pain specialist nurses on the quality of life for patients with cancer pain at home

目的探讨疼痛专科护士主导多学科协作模式对居家癌痛病人生活质量的影响。方法选取南充市某三甲综合性医院肿瘤科及疼痛科出院居家治疗的癌痛病人468例,随机分为对照组和干预组,其中对照组219例、干预组228例,对照组采取常规的出院随访,干预组采取以疼痛专科护士主导的多学科协作模式对居家治疗的癌痛病人进行管理,干预时间为3个月,主要指标为居家癌痛病人的生活质量,次要指标为居家癌痛病人的癌痛药物副作用、服药依从性、焦虑和抑郁评分、疼痛评分。结果干预组病人的生活质量评分及服药依从性均高于对照组(P<0.001),干预组癌痛药物副作用、焦虑和抑郁评分、疼痛评分均低于对照组(P<0.001)。结论在癌痛病人出院管理中采取疼痛专科护士主导多学科协作的管理模式,对于改善病人生活质量、服药依从性、癌痛药物副作用、焦虑和抑郁评分及疼痛评分等均有积极影响。

Objective To explore the effect of pain specialist nurse-led multidisciplinary collaboration model on the quality of life of patients with cancer pain at home.Methods A total of 468 patients with cancer pain who were discharged from the oncology depart⁃ment and pain department of a tertiary hospital in Nanchong City for home treatment were selected and randomly divided into a control group and an intervention group,of which 219 cases were in the control group and 228 cases in the intervention group.The control group underwent routine discharge follow-up.the intervention group adopted a multidisciplinary collaboration model led by pain spe⁃cialist nurses to manage cancer pain patients treated at home.The intervention time was 3 months.The main indicator was the patients’quality of life,and the secondary indicators were the side effects of cancer pain drugs,medication Adherence,anxiety and depression scores,and pain scores.Results The quality of life score and medication compliance of the intervention group were higher than those of the control group(P<0.001).The side effects of cancer pain drugs,anxiety and depression scores,and pain scores in the intervention group were lower than those in the control group(P<0.001).Conclusion The management mode of multidisciplinary collaboration led by pain specialist nurses in the discharge management of cancer pain patients had a positive impact on improving the quality of life,medication compliance,side effects of cancer pain drugs,anxiety and depression scores,and pain scores of patients.
