标本溶血对常规生化检验结果的影响分析

摘要 目的 分析标本溶血对常规生化检验结果的影响。方法 选取2020年9月-2021年9月信阳市息县人民医院检验科收检的138例健康体检者进行研究,抽取所选健康者的空腹静脉血5mL均分为2份,无溶血反应的血液标本设为对照组,将经过溶血处理发生溶血反应的血液标本设为干预组。检测两组肝功能检测指标、心肌酶检测指标、血脂检测指标、肾功能检测指标、电解质指标等常规生化检验指标。结果 干预组的总胆红素、直接胆红素、总蛋白、球蛋白、白蛋白、谷氨酸氨基转移酶、胆碱酯酶、天门冬氨酸氨基转移酶等肝功能检测指标水平均高于对照组,差异有统计学意义(P<0.05);干预组的肌酸激酶、羟丁酸脱氢酶、乳酸脱氢酶等心肌酶检测指标水平均高于对照组,差异有统计学意义(P<0.05);干预组的血肌酐、尿酸、尿素氮等肾功能检测指标水平均高于对照组,差异有统计学意义(P<0.05);干预组的钠、钙、钾、氯电解质指标水平均高于对照组,差异有统计学意义(P<0.05);干预组的总胆固醇水平、甘油三酯水平、低密度蛋白水平均高于对照组,差异有统计学意义(P<0.05);干预组的高密度蛋白水平低于对照组,差异有统计学意义(P<0.05)。结论 标本溶血会对常规生化检验结果造成不良影响,导致检测结果数据误差,影响临床诊断治疗,临床应注意控制、减少标本发生溶血反应。 Objective To analyze effect of sample hemolysis on routine biochemical test results.Methods The paper chose 138 healthy examiners in Laboratory Department of Xinyang City Xixian County People's Hospital from September 2020 to September 2021 for study,collected fasting venous blood 5 mL and divided into two parts equally.Blood samples without hemolysis reaction were included in control group,and blood samples with hemolysis reaction in intervention group.Routine biochemical test indicators including liver function test indicators,myocardial enzyme test indicators,blood lipid test indicators,renal function test indicators,electrolyte indicators were detected of two groups.Results Levels of total bilirubin,direct bilirubin,total protein,globulin,albumin,glutamic acid aminotransferase,cholinesterase,aspartate aminotransferase and other liver function indicators in intervention group were higher than control group,with statistically significant difference(P<0.05).Levels of myocardial enzymes including creatine kinase,hydroxybutyrate dehydrogenase,and lactate dehydrogenase in intervention group were significantly higher than control group(P<0.05).Levels of serum creatinine,uric acid,urea nitrogen and other renal function indicators in intervention group were significantly higher than control group(P<0.05).Levels of sodium,calcium,potassium,and chloride electrolytes in intervention group were significantly higher than control group(P<0.05).Levels of total cholesterol,triglycerides,and low density protein in intervention group were significantly higher than control group(P<0.05).High-density protein level in intervention group was significantly lower than control group(P<0.05).Conclusion Specimen hemolysis can cause adverse effect on results of routine biochemical tests with data errors,and affect clinical diagnosis and treatment.It is necessary to pay attention to controlling and reducing hemolytic reactions of specimens in clinical practice.
