多普勒超声检测甲状腺结节内钙化对鉴别甲状腺癌和甲状腺良性病变的价值

Value of Doppler Ultrasound Detecting Calcification in Thyroid Nodules for Distinguishing Thyroid Cancer and Benign Thyroid Lesions

摘要 目的 探究多普勒超声检测甲状腺结节内钙化对鉴别甲状腺癌和甲状腺良性病变的价值。方法 选择2017年5月-2022年5月本院接诊治疗的甲状腺结节患者100例作为研究对象,所有患者均接受彩色多普勒超声技术检测,同时以手术病理诊断结果作为金标准,计算多普勒超声诊断的灵敏度、特异性及准确率。比较良性病变患者与甲状腺癌患者超声征象表现及内部钙化情况,记录分析良性病变患者与甲状腺癌患者血流动力参数差异性。结果 100例患者的手术病理诊断结果显示,良性病变患者共60例,占比60.00%,甲状腺癌患者40例,占比40.00%。通过计算,多普勒超声技术的灵敏度为96.67%、特异性为97.50%、准确率为97.00%。甲状腺癌患者低回声占比、血流信号丰富占比率高于良性病变患者,边界清晰占比率、形态规则占比率及囊实质占比率低于良性病变患者(P<0.05)。同时,甲状腺癌患者中,微钙化占比率高于良性病变患者(P<0.05)。经过分析患者血流动力学参数结果发现,甲状腺癌患者收缩期峰值流速高于良性病变患者,阻力指数大于良性病变患者,舒张末期血流速度低于良性病变患者(P<0.05)。结论 通过多普勒超声技术检测甲状腺结节内钙化情况可用于临床诊断疾病良恶性,具有较高灵敏度、特异性,为临床进一步治疗疾病提供影像学支持,值得运用推广。 Objective To analyze effect and value of Doppler ultrasound technology for calcified diseases in thyroid nodules in differential diagnosis of thyroid benign lesions and thyroid cancer.Methods The paper chose 100 patients with thyroid nodules treated in our hospital from May 2017 to May 2022 as research objects.All patients were treated with color Doppler ultrasound technology test,sensitivity,specificity,and accuracy of Doppler ultrasound diagnosis was calculated based on gold standard of surgical pathological diagnosis results.Ultrasound manifestation and internal calcification between patients with benign lesions and thyroid cancer was was compared,difference in hemodynamic parameters between patients with benign lesions and thyroid cancer was recorded and analyzed.Results Surgical and pathological diagnosis results of 100 patients showed,there were 60 patients with benign lesions,accounting for 60.00%,and 40 patients with thyroid cancer,accounting for 40.00%.Calculation showed,sensitivity of Doppler ultrasound technology was 96.67%,specificity was 97.50%,and accuracy was 97.00%.Proportion of low echo and ric h blood flow signals of thyroid cancer patients was higher than benign disease patients,while proportion of clear boundaries,regular morphology,and cystic parenchyma was lower than benign disease patients(P<0.05).Meanwhile,proportion of microcalcifications in patients with thyroid cancer was higher than patients with benign lesions(P<0.05).After analyzing hemodynamic parameters,it was found peak systolic flow velocity of thyroid cancer patients was higher than benign disease patients,resistance index was higher than benign disease patients,end diastolic blood flow velocity was lower than benign disease patients(P<0.05).Conclusion Doppler ultrasound technology detecting calcification in thyroid nodules can be used in clinical diagnosis of benign and malignant diseases,with high sensitivity and specificity,can provide imaging support for further clinical disease treatment,which is worthy of promotio n and application.
