肩关节镜下单排与双排缝合桥修复技术治疗老年肩袖损伤的临床比较

Clinical Comparison of Single Row and Double Row Suture Bridge Repair Techniques under Arthroscopy for Elderly Rotator Cuff Injuries

目的 根据武警江苏省总队医疗骨科对老年肩袖伤的治疗特点,就肩关节镜下单排与双排缝合桥修复技术治疗方法及效果进行评估。方法 本项目选择了武警江苏省总队医疗骨科临床中心2013年6月-2022年2月收治的近40例老年肩袖伤患者作为研究对象,并根据患者的处理方法不同分成治疗组(运用双排缝合桥修复技术)和对照组(肩关节镜下单排缝合桥修复技术),每组20例,研究了两组患者术中术后UCLA评估、Constant评估、术中术后VAS疼痛评估和患者临床治疗效果。结果 比较两组患者术前的UCLA评分和Constant评分,结果表明,手术各项数据结果对比差异无统计学意义(P>0.05);比较术后各项数据分析效果,治疗组好于对照组,差别有统计学意义(P<0.05);组内患者术前后结果差异相对地也有统计学意义(P<0.05),且两组的对照显示,手术痛苦分数相比无差别(P>0.05),术后疼痛分数对比差异显著(P<0.05),同时对比两组的临床疗效显示,治疗组的临床疗优于对照组,两组的差别有统计学意义(P<0.05)。结论 针对老年肩袖伤的患者而言,肩关节镜下双排缝合桥修复技术的治疗效果更显著,更能够满足患者的治疗要求,值得在患者治疗过程中推广。

Objective Based on treatment characteristics of elderly rotator cuff injuries in medical orthopedics department of Jiangsu Provincial Armed Police Force,to evaluate treatment methods and effect of single row and double row suture bridge repair techniques under shoulder arthroscopy.Methods The project chose nearly 40 elderly patients with rotator cuff injury admitted to Medical Orthopedic Clinical Center of Jiangsu Provincial Armed Police Force from June 2013 to February 2022 as research objects,and divided them into treatment group(treated with double row suture bridge repair technology)and control group(with single row suture bridge repair technology under arthroscopy)based on their treatment methods,with 20 cases in each group.UCLA and Constant evaluations during and after surgery,intraoperative and postoperative VAS pain and clinical treatment effect were studied in both groups.Results Preoperative UCLA scores and Constant scores between two groups comparison showed,there was no statistically significant difference between surgical data results(P>0.05).Postoperative data analysis results of treatment group were better than control group,with statistically significant difference(P<0.05).Difference in preoperative and postoperative outcomes among patients within the group was statistically significant(P<0.05),comparison between two groups showed no difference in surgical pain scores(P>0.05),while difference in postoperative pain scores was significant(P<0.05).At the same time,clinical effect of treatment group was 95.00%,better than 65.00%of control group.Difference between two groups was statistically significant(P<0.05).Conclusion Double row suture bridge repair technology under arthroscopy can achieve significant treatment effect for elderly patients with rotator cuff injury,can meet treatment requirements of patients better,which is worthy of promotion in treatment process of patients.
