血液透析联合血液灌流治疗尿毒症的疗效观察

摘要 目的 观察尿毒症患者接受血液透析+血液灌注治疗后的实际效果与临床价值。方法 选择研究对象为56例尿毒症患者,就诊时间处于2021年8月-2022年8月期间,将所有资料均纳入计算机系统中分组,对照组(共纳入28例,采用血液透析治疗),观察组(共纳入28例,采用血液透析+血液灌流治疗),评定下列多项指标临床疗效、炎性因子水平、皮肤瘙痒程度、生化指标变化、大分子毒素水平。结果 观察组临床疗效为96.4%,相较于对照组临床疗效71.4%,明显更高,两组疗效对比差异明显(P<0.05);治疗前,两组β2-MG水平、hs-CRP水平无明显差异(P>0.05);治疗后,观察组上述两项指标改善效果与对照组相比明显更好,组间对比有统计学意义(P<0.05);观察组治疗后皮肤瘙痒程度明显轻于对照组,两组对比差异明显(P<0.05);对SCr、BUN、Ca^(2+)等生化指标予以检测发现,观察组生化指标调节效果优于对照组,二者对比有统计学意义(P<0.05);干预前,两组PTH、LEP指标均处于较高水平;干预后,观察组PTH水平与LEP水平明显低于对照组,两组大分子毒素水平比较差异显著(P<0.05)。结论 尿毒症治疗过程中应用血液透析+血液灌流可提高机体毒素排除率,促进疗效提升的同时,炎性因子、生化指标均得到良好改善,减少治疗后皮肤瘙痒情况发生,延长患者生存质量,是一项值得推广治疗方案。 Objective To observe actual effect and clinical value of hemodialysis+blood perfusion therapy for uremic patients.Methods The paper chose 56 patients with uremia as study objects,with treatment period from August 2021 to August 2022.All data were included in computer system and grouped into control group(28 cases,treated with hemodialysis)and observation group(28 cases,with hemodialysis and hemoperfusion).The following multiple indicators were evaluated:Clinical effect,Inflammatory factor levels,Skin itching degree,Changes in biochemical indicators,Levels of macromolecular toxins.Results Clinical effect of observation group was 96.4%,significantly higher than 71.4%of control group,difference between two groups was significant(P<0.05).Before treatment,β2-MG and hs CRP levels showed no significant difference between two groups(P>0.05).After treatment,improvement effect of above two indicators in observation group was significantly better than control group,with statistical significance(P<0.05).After treatment,skin itching degree in observation group was significantly lighter than control group,with significant difference between two groups(P<0.05).Detection of biochemical indicators including SCR,BUN,Ca^(2+)showed,observation group had better regulatory effect on biochemical indicators than control group,with statistical significance(P<0.05).Before intervention,both groups had high levels of PTH and LEP indicators.After intervention,the PTH and LEP levels in observation group were significantly lower than control group,there was significant difference in levels of macromolec ular toxins between two groups(P<0.05).Conclusion hemodialysis+hemoperfusion can improve detoxification rate of the body,improve curative effect,inflammatory factors and biochemical indicators,reduce occurrence of skin itching of uremia after treatment,and prolong life quality,which is wort hwhile treatment plan to promote.
