关节镜套在膀胱灌注化疗药物操作中预防职业暴露的应用探索

摘要 目的 探讨使用关节镜套保护进行膀胱灌注化疗药物预防职业暴露可行性研究。方法 选取2020年3月-2021年12月本院行膀胱灌注化疗药物的80例患者为研究对象,随机数字法分为试验组和对照组,每组各40例,试验组采用关节镜套保护下进行密闭式膀胱灌注化疗药物,对照组按常规采用开放式膀胱灌注化疗药物。比较两组患者膀胱灌注化疗药物飞溅和外溢情况,对干预后膀胱灌注过程中高危环节职业暴露风险发生率比较。结果 试验组职业暴露占比明显低于对照组,两组差异具有统计学意义(P<0.05)。通过对两组患者进行不良反应情况比较,试验组明显低于对照组,两组差异具有统计学意义(P<0.05)。通过进行焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,干预后,试验组患者明显优于对照组,两组差异具有统计学意义(P<0.05)。结论 使用关节镜套保护进行膀胱灌注化疗药物能有效减少环境污染,降低医护人员职业暴露风险,有利于预防职业暴露。 Objective To explore feasibility of arthroscopic protection for bladder infusion chemotherapy drugs to prevent occupational exposure.Methods The paper chose 80 patients with bladder infusion chemotherapy drugs in our hospital from March 2020 to December 2021 as research objects,and divided them into experimental group and control group randomly,with 40 cases in each group.Experimental group was treated with closed bladder infusion chemotherapy drugs under protection of an arthroscope cover,while control group with open bladder infusion chemotherapy drugs as usual.Splashing and overflow of chemotherapy drugs during bladder infusion was compared between two groups,incidence of occupational exposure risks in high-risk areas during bladder infusion was compared after intervention.Results There were 4 cases of occupational exposure in experimental group,accounting for 10.0%,and 30 cases of occupational exposure in control group,accounting for 75.0%.Difference between two groups was statistically significant(P<0.05).Chemotherapy drugs for bladder perfusion overflow in experimental group of 40 cases were limited to arthroscopic sleeve.Comparing adverse reactions between two groups,number of patients in experimental group with bladder perfusion experienced drug leak age and local discomfort was 2(5.0%)and 1(2.5%)respectively,significantly lower than control group.Difference between two groups was statistically significant(P<0.05).Self Rating Anxiety Scale(SAS)and Self Rating Depression Scale(SDS)scores in experimental group after intervention were significantly better than control group,difference between two groups was statistically significant(P<0.05).Conclusion Arthroscopic protection for bladder infusion of chemotherapy drugs can reduce environmental pollution,occupational exposure risks of medical staff effectively,which is beneficial for pre venting occupational exposure.
