内镜下电凝止血联合艾司奥美拉唑钠对上消化道出血患者止血和凝血功能的影响

Effect of Electrocoagulation Hemostasis Combined with Esomeprazole Sodium on Hemostasis and Coagulation in Patients with Upper Gastrointestinal Bleeding

目的 探讨内镜下电凝止血联合艾司奥美拉唑钠对上消化道出血患者止血和凝血功能的影响。方法 选取本院2020年1月-2022年3月收治的92例上消化道出血患者,随机分为对照组和观察组,各46例。对照组患者实施内镜下电凝止血治疗,观察组患者在对照组的基础上联合艾司奥美拉唑钠治疗,比较两组患者治疗前后凝血功能、血常规、止血有效率和再出血情况。结果 治疗前,两组患者凝血功能水平比较差异无统计学意义(P>0.05);治疗后,观察组患者APTT(30.08±2.25)s、PT(12.30±1.01)s、TT(13.16±1.12)s和FIB(2.17±0.12)g/L均低于对照组(P<0.05)。治疗前,两组患者血常规指标之间比较差异无统计学意义(P>0.05);治疗后,观察组患者红细胞计数(4.59±0.46)×10^(12)/L、血小板计数(150.13±2.42)×10^(9)/L和血细胞比容(45.71±5.37)%均高于对照组(P<0.05)。观察组患者止血有效率为97.82%,高于对照组的82.61%(P<0.05)。观察组患者再出血率为2.17%,低于对照组的15.22%(P<0.05)。结论 上消化道出血患者给予内镜下电凝止血联合艾司奥美拉唑钠治疗,可有效调节患者凝血功能,优化血常规指标,提高止血有效率,减少再出血情况。

Objective To investigate the effect of endoscopic electrocoagulation hemostasis combined with esomeprazole sodium on hemostasis and coagulation in patients with upper gastrointestinal bleeding.Methods A total of 92 patients with upper gastrointestinal bleeding who were admitted to our hospital from January 2020 to March 2022 were selected and randomly divided into the control group and the observation group,with 46 cases in each.The patients in the control group were treated with endoscopic electrocoagulation for hemostasis,and the patients in the observation group were treated with esomeprazole sodium on the basis of the control group.The coagulation function,blood routine,effective rate of hemostasis and rebleeding were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in blood coagulation function between the two groups(P>0.05);after treatment,APTT(30.08±2.25)s,PT(12.30±1.01)s,TT(13.16±1.12)in observation group)s and FIB(2.17±0.12)g/L were lower than those in the control group(P<0.05).Before treatment,there was no significant difference in blood routine indexes between the two groups(P>0.05);after treatment,the red blood cell count in the observation group was(4.59±0.46)×10^(12)/L,and the platelet count was(150.13±2.42)×10^(9)/L and hematocrit(45.71±5.37)%were higher than those in the control group(P<0.05).The effective rate of hemostasis in the observation group was 97.82%,which was higher than 82.61%in the control group(P<0.05).The rebleeding rate in the observation group was 2.17%,which was lower than 15.22%in the control group(P<0.05).Conclusion Endoscopic electrocoagulation hemostasis combined with esomeprazole sodium therapy for patients with upper gastrointestinal bleeding can effectively regulate the coagulation function of patients,optimize blood routine indexes,improve the effective rate of hemostasis,and reduce rebleeding.
