慢性病轨迹框架护理干预对高血压脑出血患者心理及自我管理能力的影响研究

Influence of Chronic Disease Trajectory Framework Nursing Intervention on Psychology and Self-management Ability of Patients with Hypertensive Intracerebral Hemorrhage

目的 探讨慢性病轨迹框架护理干预对高血压脑出血患者心理及自我管理能力的影响。方法 选取2021年3月-2022年5月本院收治的高血压脑出血患者110例,随机分为观察组与对照组,每组各55例。对照组患者接受神经外科常规护理干预,观察组在神经外科常规护理基础上实施慢性病轨迹框架护理干预。比较两组SAS评分、SDS评分、ADL评分、ESCA评分、护理满意度和并发症发生率。结果 干预前,两组SAS评分、SDS评分、ADL评分、ESCA评分无显著差异(P>0.05);干预后,观察组SAS评分、SDS评分显著低于对照组(P<0.05),ADL评分和ESCA评分在统计学上明显比对照组高一些(P<0.05)。观察组护理满意度(94.55%)显著优于对照组(78.18%)(P<0.05)。与对照组并发症发生率(29.09%)相比,观察组并发症发生率(5.45%)统计学上更低一些(P<0.05)。结论 对高血压脑出血患者应用慢性病轨迹框架干预能改善患者心理状态,提高日常生活能力和自我管理能力,降低并发症发生率。

Objective To explore the effects of chronic disease trajectory framework nursing intervention on the psychological and self-management abilities of patients with hypertensive intracerebral hemorrhage.Methods A total of 110 patients with hypertensive cerebral hemorrhage who were treated in our hospital from March 2021 to May 2022 were selected and randomly is divided into the observation series and the control series,with 55 cases in each group.The patients in the control group received the routine nursing intervention of neurosurgery nursing,and the patients in the observation group received the chronic disease trajectory framework intervention based on the routine nursing of neurosurgery nursing.The SAS,SDS,ADL,ESCA,nursing satisfaction and complication rates were compared between the two groups.Results Before interposing,the two seriess of sufferers,SAS,SDS,ADL and ESCA do not there is not marked divergence at maring(P>0.05).Interpose rear observation series SAS,SDS marks obviously is lower than the comparison series(P>0.05),and the ADL and observation series ESCA marks the obvious over comparison series(P>0.05).The nursing satisfaction of the observation series(94.55%)obviously is superior to the comparison series(78.18%)(P<0.05).The incidence of complications in the observation series(5.45%)obviously is lower than the comparison series(29.09%)(P<0.05).Conclusion The application of chronic disease trajectory framework intervention to patients with hypertensive intracerebral hemorrhage can improve the psychology condition of sufferer,improve ability of daily living and self-management,and reduce the incidence of complications.
