家庭医生签约服务在社区糖尿病患者管理中的应用

Application of Family Doctor Contract Service in the Management of Community Diabetic Patients

目的 在社区糖尿病患者管理过程中实施家庭医生签约服务,观察实际应用效果。方法 对本社区医院2020年6月-2021年6月收治的120例糖尿病患者进行回顾性分析,根据干预方案的不同分为签约组(n=60)及对照组(n=60),对照组实施常规管理,签约组在此基础上开展家庭医生签约服务,对比两组患者血糖、糖化血红蛋白、服药依从性以及服务满意度情况。结果 干预前,两组空腹血糖、餐后2h血糖以及糖化血红蛋白水平比较差异无统计学意义(P>0.05);干预1年后,两组上述指标水平均下降,但签约组各指标水平均要低于对照组(P<0.05)。干预前,两组MMAS-8评分比较差异无统计学意义(P>0.05);干预1年后,两组MMAS-8评分均上升,但签约组评分高于对照组(P<0.05)。签约组满意度(98.33%)要明显高于对照组(86.67%)(P<0.05)。结论 在社区糖尿病患者管理过程中开展家庭医生签约服务有利于稳定患者血糖水平,改善患者服药依从性,促进患者按照医嘱服药,进一步提升患者满意度。

Objective To implement family doctor contract service in the management of community diabetic patients,and to observe the actual application effect.Methods A retrospective analysis was performed on 120 diabetic patients admitted to our community hospital from June 2020 to June 2021.According to the different intervention programs,they were divided into the contract group(n=60)and the control group(n=60).The control group implemented routine management,and the contract group carried out family doctor contract service on this basis.The blood glucose,glycosylated hemoglobin,medication compliance and service satisfaction were compared between the two groups.Results Before intervention,there was no significant difference in fasting blood glucose,2h postprandial blood glucose and glycosylated hemoglobin levels between the two groups(P>0.05).After 1 year of intervention,the levels of fasting blood glucose,2h postprandial blood glucose and glycosylated hemoglobin in both groups decreased,but the levels in the contract group were lower(P<0.05).Before intervention,there was no significant difference in the MMAS-8 scores between the two groups(P>0.05).After 1 year of intervention,the MMAS-8 scores in both groups increased,but the scores in the contract group were higher than those in the control group(P<0.05).The satisfaction degree of the contract group(98.33%)was significantly higher than that of the control group(86.67%)(P<0.05).Conclusion Carrying out family doctor contract service in the process of community diabetes patient management is conducive to stabilizing patients'blood sugar level,improving patients'medication compliance,promoting patients to take medication according to doctor's orders,and further improving patient satisfaction.
