内镜微创手术治疗对微小型甲状腺癌患者临床效果和应激反应的影响

Effect of Endoscopic Minimally Invasive Surgery on the Clinical Effect and Stress Response of Patients with Mini Thyroid Cancer

目的 分析微小型甲状腺癌手术治疗的临床价值。方法 选取2020年3月-2022年3月本院收治的58例微小型甲状腺癌手术治疗患者,按照随机抽签分组原则分为对照组和观察组,各29例。对照组患者应用开放性手术治疗,观察组患者应用内镜微创手术治疗。对比两组患者临床指标、创伤应激、治疗有效率和术后并发症。结果观察组患者手术时间(42.86±8.93)min、术后引流量(72.53±4.25)mL、住院时间(5.07±1.13)d均低于对照组(P<0.05);观察组患者治疗后白细胞介素-6(5.18±0.96)mmol/L和皮质醇(312.67±24.83)nmol/L均低于对照组(P<0.05);观察组患者治疗有效率96.55%高于对照组89.66%(P<0.05);观察组患者术后并发症发生率3.45%低于对照组27.59%(P<0.05)。结论 微小型甲状腺癌患者采用内镜微创手术治疗,可缩短手术时间,减少创伤应激,降低术后并发症发生率。

Objective To analyze the clinical value of surgical treatment of small thyroid cancer.Methods From March 2020 to March 2022,58 patients with micro and small thyroid cancer treated by surgery in our hospital were selected and divided into control group and observation group according to the principle of random lottery,with 29 patients in each group.Patients in the control group were treated with open surgery,and patients in the observation group were treated with endoscopic minimally invasive surgery.The clinical indexes,traumatic stress,effective rate of treatment and postoperative complications were compared between the two groups.Results The operation time(42.86±8.93)min,postoperative drainage volume(72.53±4.25)mL and hospitalization time(5.07±1.13)days in the observation group were lower than those in the control group(P<0.05);after treatment,the levels of IL-6(5.18±0.96)mmol/L and cortisol(312.67±24.83)nmol/L in the observation group were lower than those in the control group(P<0.05);the effective rate of 96.55%in the observation group was higher than 89.66%in the control group(P<0.05);the postoperative complication rate in the observation group was 3.45%lower than 27.59%in the control group(P<0.05).Conclusion Endoscopic minimally invasive surgery can shorten the operation time,reduce the traumatic stress and reduce the incidence of postoperative complications.
