预见性护理模式对ICU机械通气患者肺部感染的预防作用

Preventive Effect of Predictive Nursing Mode on PulmonaryInfection in Patients with Mechanical Ventilation in ICU

目的 分析予以重症监护室(ICU)机械通气患者预见性护理在预防肺部感染中的作用。方法 选取2020年6月至2022年6月商丘市中医院ICU收治的76例机械通气患者,经随机数字表法将其中38例归纳到对照组(接受一般护理),余下38例归纳到观察组(于前组基础上加以预见性护理)。比较两组于不同护理模式下肺部感染发生率及死亡率,感染持续时间、机械通气时间及入住ICU时间,其他并发症出现情况,对护理工作的满意度评分。结果 观察组肺部感染率及死亡率低于对照组,观察组感染持续、机械通气及入住ICU时间均短于对照组,观察组压疮、非计划拔管、面部皮肤损伤、呼吸道阻塞及低蛋白血症占比均低于对照组,观察组护患沟通、健康宣教、护理态度、护理技术及护理效果满意度评分和总分均高于对照组,差异有统计学意义(P <0.05)。结论 予以ICU机械通气患者预见性护理能降低其死亡率,缩短其感染持续时间、机械通气时间和入住ICU时间,减少肺部感染等并发症出现,提升患者满意度。

Objective To analyze the prevention effects of predictive nursing mode on pulmonary infections in patients with mechanical ventilation in the ICU.Methods A total of 76 mechanically ventilated patients admitted to the ICU of Shangqiu Hospital of Traditional Chinese Medicine from June 2020 to June 2022 were selected.By the random number table method,38 cases were classified into the control group(receiving general nursing),and the remaining 38 cases were classified into the observation group(receiving predictive nursing on the basis of the former group).The incidence and mortality of pulmonary infection,duration of infection,mechanical ventilation time and ICU stay time,the occurrence of other complications,and the satisfaction score of nursing work were compared between the two groups under different nursing modes.Results The rate of pulmonary infection and mortality in the observation group was lower than that in the control group.The duration of infection,mechanical ventilation,and ICU stay time in the observation group were shorter than those in the control group.The rates of pressure sore,unplanned extubation,facial skin injury,respiratory obstruction,and hypoproteinemia in the observation group were lower than those in the control group.The nursing patient communication,health education,nursing attitude,nursing technology,and nursing effect satisfaction scores and total scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Predictive nursing can reduce the mortality rate,shorten the duration of infection,mechanical ventilation time and ICU stay time,reduce the occurrence of lung infection and other complications,and improve patient satisfaction.
