IMCHB下责任制全局护理模式对老年肺癌患者自我效能水平及生活质量的影响

Effect of an Accountable Global Care Model Under IMCHB on theLevel of Self-Efficacy and Quality of Life in Elderly Patients withLung Cancer

目的 探讨健康行为互动(IMCHB)下责任制全局护理模式在老年肺癌患者中的应用效果。方法 选取郑州大学第一附属医院2020年1月至2022年1月收治的101例老年肺癌患者,根据入院顺序分为研究组(N=51)和常规组(N=50)。常规组采用常规护理干预,研究组在常规组基础上联合IMCHB下责任制全局护理模式,比较两组自我效能[自我管理效能问卷(SUPPH)]、情绪状态[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、生活质量、健康行为、护理满意度。结果 干预后研究组SUPPH评分高于常规组,SAS、SDS评分低于常规组,且干预后研究组生理状况、情感状况、社会/家庭状况、功能状况评分均高于常规组,差异有统计学意义(P <0.05)。研究组健康行为优良率[86.27%(44/51)]高于常规组[62.00%(31/50)],护理满意度[96.08%(49/51)]高于常规组[80.00%(40/50)],差异有统计学意义(P <0.05)。结论 IMCHB下责任制全局护理模式可提高老年肺癌患者自我效能,缓解患者负性情绪,改善患者健康行为,提升患者生活质量及护理满意度。

Objective To investigate the effect of an accountable global care model under the interaction model of client health behavior(IMCHB)in elderly patients with lung cancer.Methods A total of 101 elderly patients with lung cancer admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to January 2022 were selected and divided into study groups(N=51)and routine groups(N=50)according to the admission order.Routine nursing intervention was used in the conventional group,the research group was combined with an accountable global care model under IMCHB on the basis of the conventional group.The self-efficacy[strategies used to promote health(SUPPH)],emotional state[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],quality of life,health behavior,and nursing satisfaction of the two groups were compared.Results After the intervention,the SUPPH score of the study group was higher than that of the conventional group,and the SAS and SDS scores were lower than that of the conventional group.After the intervention,the scores of physiological status,emotional status,social/family status,and functional status of the study group were higher than that of the conventional group,with statistical significance(P<0.05).The study group had a higher rate of excellent health behavior 86.27%(44/51)than the conventional group 62.00%(31/50),and the nursing satisfaction[96.08%(49/51)]was higher than that in the conventional group[80.00%(40/50)],the difference was statistically significant(P<0.05).Conclusion The accountable global care model under IMCHB can improve the self-efficacy of elderly lung cancer patients,relieve patients'negative emotions,improve patients'health behaviors,and enhance patients'quality of life and nursing satisfaction.
