六字诀对脑卒中后疲劳的疗效观察及气虚血瘀证候的影响

Efficacy of Liuzijue on post-stroke fatigue of Qideficiency syndrome and blood stasis

目的观察六字诀对脑卒中后疲劳患者的疗效及气虚血瘀证候的影响。方法选取2022年6月至2023年3月在福建中医药大学附属康复医院住院的脑卒中后疲劳患者68例,按照随机数字表分为治疗组和对照组各34例。对照组采用常规康复治疗与常规呼吸治疗,治疗组采用常规康复治疗与六字诀,30 min/次,1次/d,5 d/周,共进行12周。干预前后采用疲劳严重度量表(fatigue severity scale,FSS)、气虚血瘀证候要素诊断量表、呼吸困难程度分级[改良版的英国医学委员会量表(mMRC)]评分进行评估。结果最终完成64例,其中治疗组32例,对照组32例。与干预前对比,两组干预后,治疗组FSS评分(2.875±0.707)分与气虚血瘀证候要素诊断量表评分(20.219±5.204)分、(14.656±3.488)分改善程度明显优于对照组(4.313±0.471分)、(27.438±5.315)分、(22.313±4.432)分,差异有统计学意义(P<0.05);两组mMR C评分均较治疗前降低,治疗组与对照组比较,差异无统计学意义(P>0.05)。结论六字诀能够改善脑卒中后疲劳患者的疲劳严重度与气虚血瘀证候评分,改善患者的疲劳症状,且促进患者脏腑气血运行。

Objective To observe the effect of Liuzijue on post-stroke fatigue of Qi deficiency syndrome and blood stasis.Methods From June 2022 to March 2023,a total of 68 post-stroke fatigue patients who visited the Affiliated Rehabilitation Hospital of Fujian University of Traditional Chinese Medicine were enrolled in this study.the patients were randomly assigned into therapy group(n=34)and control group(n=34).The control group was treated with conventional rehabilitation combined with conventional respiratory therapy.The therapy group was treated with Liuzijue combined with conventional rehabilitation,30 minutes/time,5 times/week once daily(QD)for 12 weeks.Before and after the intervention,fatigue severity scale(FSS),Qi deficiency syndrome and blood stasis syndrome factor diagnostic scale,and modified medical research council(mMRC)dyspnea scale were used for evaluation.Results Finally,64 cases were completed,including 32 in the therapy group and 32 in the control group.Compared with that before intervention,the improvement of FSS score(2.875±0.707)points and the diagnostic scale score(20.219±5.204)points,(14.656±3.488)points in the therapy group were significantly better than the control group(4.313±0.471)points,(27.438±5.315)points,(22.313±4.432)points(P<0.05).mMRC scores in both groups were lower than before treatment,and there was no significant difference in mMRC scores between groups after treatment(P>0.05).Conclusion Patients with post-stroke fatigue of Qi deficiency syndrome and blood stasis,Liuzijue can reduce the fatigue severity and chinese medicine syndrome scores,improve their fatigue symptoms,and promote the movement of viscera and Qi and blood of patients,which is worthy of clinical application.
