颅脑外伤合并视神经损伤的临床分析

Clinical analysis of the craniocerabral trauma with optic nerve damage

目的 :分析颅脑外伤患者中合并视神经损伤的情况 ,以利诊断、治疗。方法 :对 72 7例颅脑外伤患者中合并有视神经损伤者 3 7例进行回顾性总结 ,就其发生率 ,损伤的机理 ,临床表现 ,诊断和治疗进行了分析。结果 :72 7例颅脑外伤患者中 12 2例合并有各种眼外伤 ,包括眼球破裂 ,眶骨骨折 ,球后血肿 ,等等。合并有视神经损伤者 3 7例 ,其中视神经断裂 9例 ,视神经挫伤 2 8例 ,占 72 7例颅脑外伤的 0 44 % ,占合并有眼外伤 12 2例的 3 0 3 2 %。 18例失明 ,其余 19例均有不同程度视力障碍及视野缺损或偏盲。结论 :在颅脑外伤患者的治疗中应该重视眼科和神经外科医生的密切配合 ,抓住视神经损伤的最佳救治时机。

Objective:To evaluate the situation of the cases of craniocerebral damage with optic nerve injury. Methods:37 cases of optic nerve injury in 727 cases of craniocerebral damage were analyzed retrospectively.The incidence rate,mechanism of injury,clinical expression,diagnosis and treatment were studied. Results:122 cases with various ocular injuries in 727 cases of craniocerebral damage included eyeball rupture,orbital fracture,postbulbar hematoma,and so on.37 cases were with optic nerve injuries.The incidence rate was 0 44% and 30 32% in craniocerebral damage and ocular injury group,respectively. Conclusions:The patient with craniocerebral damage complicated optic nerve injury shall be given ophthalmologic treatment as soon as possible.
