新疆医院精准药学服务现状调研

Investigation on the Current Status of Precision Pharmaceutical Care in Xinjiang Hospitals

目的推动新疆医院精准药学服务发展。方法于2022年3月至5月以问卷调查形式调研新疆69家医院(包括三级医院31家,二级医院35家,一级医院3家)的696名医务人员,了解各院精准药学服务开展现状,分析存在的问题及医务人员对精准药学服务的了解程度。结果共发放调查问卷696份,均有效回收,仅58.06%的三级医院和8.57%的二级医院开展精准药学服务(共21家医院)。专业技术人才缺失,医院领导不够重视,临床对检测结果接受程度不高是其余医院未开展该服务的主要原因。开展该服务的技术手段主要为治疗药物监测及药物基因组学检测。其中仅14.29%的医院年均检测量多于1000例次(均为三级医院);仅12.36%(86/696)的医务人员对精准药学服务非常了解。检测费用高、医务人员了解程度低、检测结果不能完全指导临床等是69家医院精准药学服务开展中存在的主要问题。结论对于精准药学服务,建议相关行政部门将其项目纳入医保支付范围,以解决检测费用高的问题;医院应加大宣传力度,加强医务人员培训并激励其参与相关课题研究;建立精准医疗研究团队,培养多学科交叉的专业人才;建立覆盖新疆的数据共享平台,实现信息和资源共享,推动当地精准药学服务发展。

Objective To promote the development of precision pharmaceutical care in Xinjiang hospitals.Methods From March to May 2022,a questionnaire survey was conducted on 696 medical personnel from 69 hospitals in Xinjiang(including 31 tertiary hospitals,35 secondary hospitals and three primary hospitals)to understand the current status of developing precision pharmaceutical care in hospitals,the existing problems and the medical personnel's understanding level for precision pharmaceutical care were analyzed.Results A total of 696 questionnaires were distributed,all of which were effectively collected.Only 58.06%of tertiary hospitals and 8.57%of secondary hospitals developed the precision pharmaceutical care(a total of 21 hospitals).The lack of professional technical talents,insufficient attention of hospital leaders and low-level acceptance for test results in clinical practice were the main reasons why the other hospitals did not develop this care.The main technical means to develop precision pharmaceutical care were therapeutic drug monitoring and pharmacogenomics testing.Only 14.29%of hospitals(all of them were tertiary hospitals)had an average annual testing over 1000 cases,and only 12.36%(86/696)of medical personnel had a good understanding for precision pharmaceutical care.The main problems of developing precision pharmaceutical care in 69 hospitals were high testing costs,low-level understanding of medical personnel for this care and the inability of testing results to fully guide clinical practice.Conclusion Relevant administrative departments should include the costs related to precision pharmaceutical care in the scope of medical insurance payment to solve the problem of high testing costs.Hospitals should increase their publicity,strengthen the training of medical personnel,and encourage them to participate in the relevant research projects.We should establish a precision medical research team and cultivate interdisciplinary professional talents.We should establish a data sharing platform covering Xinjiang to realize the information sharing,resource intercommunication and promote the development of local precision pharmaceutical care.
