石氏逐痰通络汤配合三色敷药治疗腰椎间盘突出症临床观察

Clinical Observation on Shi’s Zhutan Tongluo Decoction Combined with Tricolor Medicinal Paste in the Treatment of Lumbar Disc Herniation

目的观察石氏逐痰通络汤配合三色敷药治疗腰椎间盘突出症的临床效果及安全性。方法将200例腰椎间盘突出症患者随机分为对照组、中药组、膏药组和中药+膏药组,每组50例,分别给予塞来昔布胶囊、逐痰通络汤、三色敷药、逐痰通络汤联合三色敷药治疗。治疗4周后,观察临床疗效,比较视觉模拟量表(VAS)评分、日本骨科学会下腰痛评分表(JOA)评分的变化情况,评价其安全性。结果最终完成试验者196例;中药+膏药组临床疗效优于对照组、中药组和膏药组(P<0.05);4组疼痛VAS评分均明显降低(P<0.05),中药+膏药组在治疗4周后VAS评分降低显著(P<0.05);4组JOA评分均明显升高(P<0.05),中药+膏药组在治疗3周、4周后JOA评分升高显著(P<0.05);4组不良反应发生率比较差异无统计学意义(P>0.05)。结论石氏逐痰通络汤配合三色敷药治疗腰椎间盘突出症效果显著,安全性好,值得推广。

Objective To observe the clinical effect and safety of Shi’s Zhutan Tongluo decoction combined with tricolor medicinal paste in the treatment of lumbar disc herniation(LDH).Methods 200 patients with LDH were randomly divided into control group,Chinese medicine group,plaster group,Chinese medicine and plaster group,with 50 cases in each group.They were treated with celecoxib,Zhutant Tongluo decoction,tricolor medicinal paste,Zhutan Tongluo decoction combined with tricolor medicinal paste,respectively.After four weeks of treatment,the clinical effect was observed,the changes of VAS score and JOA lumbar function score were compared,and the safety of comprehensive treatment was evaluated at the same time.Results 196 patients completed the experiment.The clinical effect of Chinese medicine and plaster group was better than that of control group,Chinese medicine group and plaster group(P<0.05).Pain VAS score of 4 groups was significantly decreased(P<0.05),and VAS score of Chinese medicine and plaster group was significantly decreased after 4 weeks of treatment(P<0.05).JOA score in 4 groups was significantly increased(P<0.05),and JOA score in Chinese medicine and plaster group was significantly increased after 3 weeks and 4 weeks of treatment(P<0.05).There was no significant difference in the incidence of adverse reactions among the 4 groups(P>0.05).Conclusion Shi’s Zhutan Tongluo decoction combined with tricolor medicinal paste in the treatment of LDH is effective,safe and worthy of promotion.
