单分支支架植入术结合开窗技术治疗De BakeyⅢ型主动脉夹层的临床效果观察

Observation on the Clinical Effect of Single Branch Stent Implantation Combined with Fenestration Technique in the Treatment of DeBakey Ⅲ Aortic Dissection

目的探讨单分支支架植入术结合开窗技术治疗DeBakeyⅢ型主动脉夹层的临床效果。方法选取2017年12月至2021年6月我院收治的70例DeBakeyⅢ型主动脉夹层患者,随机分为两组各35例。对照组采用单分支支架植入术治疗,观察组采用单分支支架植入术结合开窗技术治疗。比较两组的围术期指标、并发症发生率及术后随访期间的预后情况。结果与对照组相比,观察组手术时间较长,术中出血量较低,住院时间较短(P<0.05)。观察组并发症发生率及术后随访期间不良预后发生率均低于对照组(P<0.05)。结论与单分支支架植入术治疗相比,单分支支架植入术结合开窗技术治疗DeBakeyⅢ型主动脉夹层效果更好,可明显促进患者病情康复,降低并发症发生率,改善患者预后结局。

Objective To explore the clinical effect of single branch stent implantation combined with fenestration technique in the treatment of DeBakey Ⅲ aortic dissection.Methods 70 patients with DeBakey Ⅲ aortic dissection admitted to our hospital from December 2017 to June 2021 were selected and randomly divided into two groups,with 35 cases in each group.The control group was treated with single branch stent implantation,and the observation group was treated with single branch stent implantation combined with fenestration technique.The perioperative indicators,incidence of complications and prognosis during postoperative follow-up were compared between the two groups.Results Compared with the control group,the operation time of the observation group was longer,the intraoperative blood loss was lower,and the hospitalization time was shorter(P<0.05).The incidences of complications and poor prognosis during postoperative follow-up in the observation group were lower than those in the control group(P<0.05).Conclusions Compared with single branch stent implantation,single branch stent implantation combined with fenestration technique has better effect in the treatment of DeBakey Ⅲ aortic dissection,which can significantly promote the rehabilitation of patients,reduce the incidence of complications,and improve the prognosis.
