摘要
目的探讨活动性肺结核早期检验中荧光定量检验PCR与涂片抗酸染色法的价值。方法80例肺结核患者入院后均收集痰标本分别进行荧光定量检验PCR与涂片抗酸染色法检测。以临床诊断结果为金标准,比较两种诊断方法对活动性肺结核的诊断效能。采用κ检查分析荧光定量检验PCR及涂片抗酸染色法与金标准的一致性。结果80例肺结核患者经临床诊断确诊活动性肺结核70例,非活动性肺结核10例。κ检验分析结果显示,荧光定量检验PCR检查结果与金标准的一致性较好(κ=0.940,P<0.05);涂片抗酸染色法检查结果与金标准的一致性中等(κ=0.591,P<0.05)。荧光定量检验PCR诊断活动性肺结核的灵敏度(100.00%vs.75.71%)、特异度(90.00%vs.20.00%)及准确度(98.75%vs.68.75%)均高于涂片抗酸染色法(P<0.05)。结论荧光定量检验PCR对活动性肺结核早期检验的诊断效能较高,适合作为该类疾病临床鉴别诊断的优先选择。
Objective To explore the value of fluorescence quantitative PCR and acid-fast staining in the early detection of active pulmonary tuberculosis.Methods Sputum samples were collected from 80 cases of patients with pulmonary tuberculosis after admission for fluorescent quantitative PCR and acid-fast staining.Taking clinical diagnosis results as the gold standard,the diagnostic efficacy of the two diagnostic methods for active pulmonary tuberculosis was compared.k test was used to analyze the consistency of fluorescence quantitative PCR and acid-fast staining with the gold standard.Results 70 cases of active pulmonary tuberculosis and 10 cases of inactive pulmonary tuberculosis were confirmed by clinical diagnosis.k test analysis results showed that the fluorescence quantitative PCR results had good consistency with the gold standard(k=0.940,P<0.05).The acid-fast staining results had moderate consistency with the gold standard(k=0.591,P<0.05).The sensitivity(100.00%vs.75.71%),specificity(90.00%vs.20.00%)and accuracy(98.75%us.68.75%)of fluorescence quantitative PCR were higher than those of acid-fast staining(P<0.05).Conclusions Fluorescence quantitative PCR has higher diagnostic efficiency in the early detection of active pulmonary tuberculosis,and is suitable for the clinical differential diagnosis of this disease.
