丙戊酸钠与托吡酯单药治疗小儿癫痫时对患儿脑电图放电情况、糖脂代谢的影响分析

摘要 目的:探讨丙戊酸钠、托吡酯分别单药治疗小儿癫痫对患儿脑电图放电情况、糖脂代谢的影响。方法:收集2018年2月—2020年2月我院收治的91例癫痫患儿,随机分为对照组(n=45)给予托吡酯治疗和观察组(n=46)给予丙戊酸钠治疗。比较两组患者治疗效果、免疫功能指标、脑电图放电情况、认知功能评分、糖脂代谢指标及不良反应发生情况。结果:观察组总有效率为95.7%,高于对照组的73.3%(P<0.05);治疗后观察组免疫功能优于对照组(P<0.05);治疗后观察组40 min内癫痫样放电数目及1 min内α、β、δ、θ不同频域波数目均明显低于对照组(P<0.05);治疗后观察组认知功能评分均高于对照组(P<0.05);治疗后观察组糖脂代谢指标均优于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:丙戊酸钠、托吡酯分别单药治疗小儿癫痫各具优势,临床用药中需严格控制剂量,以降低认知损害、糖脂代谢影响。 Objective:This study is aimed to investigate the effects of sodium valproate and topiramate on electroencephalogram discharge and glucose and lipid metabolism in children with epilepsy.Methods:According to the random number table method,91 cases of epileptic children admitted to our hospital from February 2018 to February 2020 were divided into two groups.The control group(n=45)was treated with sodium valproate,while the observation group(n=46)was treated with topiramate.The therapeutic effects of the two groups were compared.Results:The total effective rate in the observation group was 95.7%,which was higher than 73.3%in the control group(P<0.05);After treatment,the immune function of the observation group was better than that of the control group(P<0.05);The number of epileptic discharges within 40 minutes and within 1 minute in the observation group after treatmentα、β、δ、θThe number of waves in different frequency domains was significantly lower than that in the control group(P<0.05);After treatment,the cognitive function scores in the observation group were higher than those in the control group(P<0.05);After treatment,the indicators of glucose and lipid metabolism in the observation group were better than those in the control group(P<0.05);There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Sodium valproate and topiramate have their respective advantages in the treatment of pediatric epilepsy,and the dosage should be strictly controlled in clinical use to reduce cognitive impairment and the effects of glucose and lipid metabolism.
