地屈孕酮联合二甲双胍治疗多囊卵巢综合征的效果观察

Observation of therapeutic effect of dydrogesterone combined with metformin on polycystic ovary syndrome

目的探讨地屈孕酮联合二甲双胍治疗多囊卵巢综合征的效果。方法抽取濮阳市安阳地区医院2020年5月至2022年5月收治的多囊卵巢综合征患者96例,按照随机数字表法分为二甲双胍组与地屈孕酮组,每组48例。地屈孕酮组采用地屈孕酮治疗,二甲双胍组采用二甲双胍联合地屈孕酮治疗。比较两组临床疗效、雌二醇(E2)、卵泡刺激素(FSH)、总胆固醇(TC)、三酰甘油(TG)、空腹血糖(FPG)、空腹胰岛素(FINS)、体质指数(BMI)及血清瘦素(LEP)。结果二甲双胍组总有效率(93.75%,45/48)高于地屈孕酮组(79.17%,38/48),差异有统计学意义(χ^(2)=4.36,P=0.037)。治疗后,二甲双胍组E2、FSH水平均低于地屈孕酮组(P均<0.05)。治疗后,二甲双胍组TC、TG水平均低于地屈孕酮组(P均<0.05)。治疗后,二甲双胍组FPG、FINS水平均低于地屈孕酮组(P均<0.05)。治疗后,二甲双胍组BMI、LEP水平均低于地屈孕酮组(P均<0.05)。结论地屈孕酮联合二甲双胍治疗多囊卵巢综合征可提高临床疗效,调节糖脂代谢,减轻肥胖,改善卵巢功能。

Objective To investigate the effect of dydrogesterone combined with metformin in the treatment of polycystic ovary syndrome.Methods A total of 96 patients with polycystic ovary syndrome treated in Anyang District Hospital of Puyang from May 2020 to May 2022 were selected,and they were divided into metformin group and dydrogesterone group according to the random number table method,with 48 cases in each group.The dydrogesterone group was treated with dydrogesterone,and the metformin group was treated with metformin combined with dydrogesterone.The clinical efficacy,estradiol(E2),follicle-stimulating hormone(FSH),total cholesterol(TC),triacylglycerol(TG),fasting plasma glucose(FPG),fasting insulin(FINS),body mass index(BMI)and leptin(LEP)were compared between the two groups.Results The total effective rate of the metformin group(93.75%,45/48)was higher than that of the dydrogesterone group(79.17%,38/48),and the difference was significant(χ^(2)=4.36,P=0.037).After treatment,the levels of E2 and FSH in the metformin group were lower than those in the dydrogesterone group(all P<0.05).After treatment,the levels of TC and TG in the metformin group were lower than those in the dydrogesterone group(all P<0.05).After treatment,the levels of FPG and FINS in the metformin group were lower than those in the dydrogesterone group(all P<0.05).After treatment,the levels of BMI and LEP in the metformin group were lower than those in the dydrogesterone group(all P<0.05).Conclusions Dydrogesterone combined with metformin in the treatment of polycystic ovary syndrome can improve clinical efficacy,regulate glucose and lipid metabolism,reduce obesity,and improve ovarian function.
